Esorubicin was administered to 12 patients with soft tissue sarcoma, 1 osteosarcoma and 1 desmoid tumor. Seven patients had never received chemotherapy before. Myelotoxicity was the main side effect, leukopenia being more pronounced than thrombocytopenia. No significant nonhematological toxicity occurred. Six patients had no change of median duration of 98 days and 7 patients progressed. The patient suffering from multiple-site lesions of a desmoid tumor obtained a long-lasting partial response. This study does not support further testing of esorubicin in sarcoma patients.

Ninety-two patients with D2 prostatic cancer were studied. Initial treatment was with either diethylstilboestrol (DES) or orchidectomy. Response to DES (5/63, 81%) was significantly higher than for orchidectomy (18/29, 62%; p <0.01). However, duration of response and duration of survival were not significantly different for the 2 forms of hormonal therapy. Fifty-seven patients were randomised to receive second-line treatment with either medroxyprogesterone acetate (MPA), oral chlorambucil or combination chemotherapy (adriamycin + cyclophosphamide + 5-fluoro-uracil). Response to second-line treatment was similar for all 3 regimens (46% overall response). Most of the responses were disease stabilisation and, although there was symptomatic benefit, response to second-line therapy did not significantly improve survival compared to the survival experience of the group as a whole. It is concluded that palliative second-line treatment for advanced prostatic cancer should consist of the least toxic form of treatment which in this study was second-line hormone administration (MPA).

It has been pointed out that hepatocellular carcinoma (HCC) develops more frequently in cirrhotic liver with siderosis than in liver without iron deposition, that excess copper in hepatocytes inhibits hepatocarcinogenesis, and that an increase in copper and a decrease in zinc are seen in the sera of patients with various malignant tumors. Iron, copper and zinc concentrations in the serum and liver were estimated in normal subjects and cirrhotic patients with and without HCC. Serum copper level was significantly higher in cirrhotic patients with or without HCC than in normal subjects. No significant differences were observed in the levels of these trace elements in the serum and liver of cirrhotic patients with and without HCC. The current study seems to indicate that iron, copper and zinc do not play an important role in the development of HCC in cirrhotic patients in Japan.

The clinical course of 186 orchiectomized patients with testicular non-seminoma clinical stage I (CSI) was reviewed together with a revaluation of the haematoxylin-eosin-stained histological sections of the primary tumour. Treatment (1970–1980) was as follows: abdominal radiotherapy (group 1): 132 patients; observation only (group 2): 23 patients; retroperitoneal lymph node dissection (group 3): 31 patients. Failure of the management (diagnostic error/relapse after treatment) was defined as demonstration of retroperitoneal lymph node metastases (group 3) or relapse during follow-up (group 2 and 3). Ten of the 31 primarily operated patients had retroperitoneal lymph node metastases. (These patients received 3–4 cycles adjuvant cisplatin based on chemotherapy.) Forty-six patients relapsed after a median time of 6 months (range: 2–113). The 10-year cancer-related survival rates for group 1 and 2 were 85 and 73%, respectively. No cancer-related death occurred in group 3 within the first 5 years. In a univariate analysis the following parameters were significantly (p <0.05) correlated with management failure: vascular invasion (blood, lymphatic), the presence of syncytio-trophoblasts, the demonstration of the histological subtype MTU. In a multivariate analysis both lympatic and blood vessel invasion significantly predicted management failure. In 80% of the non-seminoma patients with CSI and both lymphatic and blood vessels invasion in the primary tumour failure of the primary management must be expected. In the multivariate analysis the demonstration of MTU was a predictive factor only if no differentiation was made between lymphatic and blood vessel invasion. The presence or absence of endodermal sinus tumour was not correlated with the failure rate in non-seminoma CSI. Based on easily assessable histopathological parameters of the primary tumour (small vessel invasion in particular) it is possible to define subgroups of patients with non-seminoma CSI who have a high or a low risk of relapse or who are likely to have microscopic retroperitoneal lymph node metastases. Effects of trilostane (TLS) on 7,12-dimethylbenz[α]anthracene (DMBA)-induced mammary cancers and body weight of rats were examined in relation to estrogen receptors (ER) in 103 female Sprague-Dawley rats with DMBA-induced cancers. The inhibitory effects of the treated groups on the growth of DMBA-induced rat mammary cancers were significantly stronger than those of the control group at many points during the treatment, demonstrating a more remarkable inhibitory effect in the high-dose groups of 300 and 500 mg/kg. In the high-dose groups, the inhibitory effects of TLS tended to be slightly stronger in the ER-negative groups than in the ER-positive groups. These results suggest that the mechanisms of action of TLS are not influenced by the presence or absence of ER, but are due to complicated hormonal changes induced by the inhibition of steroid biosynthesis

We report the cytogenetic findings observed in the cancer cells of a Wilms’tumor in a 11-month-old girl without the phenotype of the WAGR. Two populations were identified: one had a 46,XX,del(11)(p11p13) karyotype and the other had a translocation involving the deleted chromosome 11 and chromosome 12, t(11;12)(p11;q13). Cryosections of nasopharyngeal tissue from 9 patients suspected of having nasopharyngeal carcinoma (NPC) were examined for the presence of Epstein-Barr virus (EBV) markers in situ to assess virus infection in the nasopharynx. Viral DNA, EB nuclear antigen, and/or early antigens (EA.D and EA.R) were detected in 5 NPC specimens. EBV infection was not confined to the tumor areas of the biopsy specimens. Lymphoid cells and nontumor areas of these specimens contained EBV markers. In addition, nasopharyngeal tissues obtained from 3 of 4 patients in clinical remission for the disease showed evidence of EBV infection.

About one third of patients with various malignant diseases were found to have extractable amounts of DNA in their plasma whereas no DNA could be detected in normal controls. Using the test established by one of us (M. B.), which is based on decreased strand stability of cancer cell DNA, we have found that several plasma DNA originate from cancer cells.

The amount and location of interstitial collagen in the endometrial stroma and the uterine wall as well as the effects of hormonal stimulation and neoplastic transformation were studied by light and electron microscopy and immunohistochemical methods using specific antibodies to aminoterminal propeptide type III procollagen (PIIIP). The results showed collagen deposition in atrophic mucosae and in the proliferative endometrium with a decrease in the secretory endometrium. In cystic glandular hyperplasia and adenomatous hyperplasia, PIIIP deposits were abundant. In well-differentiated endometrial adenocarcinoma, dense collagen type III aggregates were found in subepithelial positions, disintegrating with advancing malignancy and decreasing in hormonal-treated morphologically well-differentiated neoplasms. The even distribution of PIIIP around muscle cells and bundles in leiomyomas was disturbed and partly absent in leiomyosarcomas, and surrounded epithelial islets in mixed mesodermal tumors.
