10 evaluable patients with metastatic hypernephroma received vinblastine (6 mg/m2, i.v., on days 1 and 8), cyclophosphamide (100 mg/m2, p.o., on days 1–14), and 5-fluorouracil (600 mg/m2, i.v., on days 1 and 8), cycled every 29 days. 7 of the patients also received dexamethasone (16 mg/day, p.o.), shortly before or during chemotherapy, in an attempt to improve their clinical status. No complete or partial responses were seen. 5 patients (50%) remained stable (median duration, 4.5 months; median survival, 6.5 months), and the remaining 5 patients progressed. A subjective response was observed in all stable patients, who also received dexamethasone. The side-effects of VEF chemotherapy were mild: 2/10 patients developed leukopenia (WBC below 3,000), 3/10 patients manifested thrombocytopenia (platelets below 100,000), and 3/10 patients alopeci

A prospective study of the hematologic findings in 19 patients receiving cis-platinum (C-P) bleomycin (B) for squamous cell carcinoma of the head and neck was performed. CP 3 mg/kg was given on day 1, B 15 mg/m2 on day 3 and then by continous infusion from day 3 to 10. Hematocrits declined in all patients from a mean of 41.6 ± 3.9 to 36.6 ± 4.9 (p < 0.001) with the nadir seen by 10 days, reticulocyte count was unchanged. In 1 patient marked hemolysis occurred over 10-day period associated with the appearance of irregular spherocytes on smear, a situation similar to the index case which initiated this study. 5 other patients had spherocytes. Ultrastructural and clinical investigations were carried out on skin metastases in patients with advanced breast cancer, following intralesional therapy with MER/BCG vaccine. A total of 65 tumoral lesions in 5 patients were injected with MER. Clinical responses were correlated with light and electron microscopic observations. Regressing tumor lesions showed substantial macrophage and lymphocyte infiltration and severe damage to tumor cells. Activated macrophages with secondary lysosomes and contacts between macrophages and tumor cells were widely observed. In 1 patient with limited cellular immune response, clinical regression was not evident and many tumor cells were ultrastructurally intact.

Peculiar inclusions, which we call B-bodies, have been observed in cells collected by leukapheresis from patients with acute leukemia. B-bodies, which often have virus-like features, were present in blast cells of 24 (44%) of the 54 patients examined: in 16 out of 29 with acute myelogenous leukemia, 5 out of 16 with acute lymphoblastic leukemia, 2 out of 3 with undifferentiated leukemia, and 1 out of 2 with chronic myelogenous leukemia in blastic transformation. The ultrastructure of these apparently polymorphic inclusions was always based on a system of cytoplasmic channels containing dense, axial filaments. Patients with B-bodies were mostly women near menopause (70% of the positives).

Recurrence occurred earlier in a small group of patients with node-negative breast carcinoma with oestrogen receptor (ER)-negative tumours, than in those in whom the tumour contained ER. The difference, indicative of an intrinsic advantage of an ER-positive tumour, was not seen following adjuvant chemotherapy. This finding could be explained on the basis of changes in the immunological balance between host and tumour brought about by treatment.

Serum levels of AFP, CEA, hCG, hPL and SP1 were measured by specific radioimmunoassays in 111 patients with testicular germ cell tumors. Seminomas, mature teratomas and ‘pure type’ embryonal carcinomas, as well as the latter two types of tumor with seminomatous admixture, do not produce markers unless in advanced stages when they may do so (small amounts of hCG, hPL and SP1). Tumors composed of yolk-sac elements alone or mixed with embryonal carcinoma produce AFP; of syncytiotrophoblastic elements – hCG, hPL or SP1; and teratomas with differentiated structures- CEA. Compound tumors can produce any of the five markers. When present in serum after orchiectomy or lymphadenectomy, the markers are useful both in diagnosis of the tumor elements that metastasized and in staging; whereas their absence does not exclude regional or distant metastases which may contain only marker-negative elements, e.g., due to changes in tumor histology. Measurement of the serum levels of the markers informs about the remaining regional tumor elements or latent metastases and therefore is more useful than immunoperoxidase staining which provides information on the already dissected structures

The data obtained with routine biochemical, immunological and haematological tests in healthy blood donors and in patients with lung cancer were subjected to discrimination analysis with the aim to select the minimum possible combination of methods with the highest probability of distinguishing between the two groups. Transformation of lymphocytes (TR.LY) after PHA stimulation, formation of E-rosettes (E-ROS), and blood glycoproteins (GP) were shown to permit the most complete differentiation between the group of patients and the blood donors. On the basis of the above-mentioned tests the discrimination rule could be defined: If 52.73249 –– 0.45913 × E-ROS –– 0.40752 × TR.LY + 0.02790 × GP ≥ 0, then the investigated person is classified as a cancer patient while at values < 0 the person is classified as a health

20 germ cell tumours have been studied with respect to the presence of alpha-fetoprotein (AFP), using the peroxidase-antiperoxidase (PAP) technique. 6 of the 20 tumours contained elements of endodermal sinus tumour (EST) and were AFP positive. 16 tumours were diagnosed either as pure embryonal carcinomas (6) or as mixed germ cell tumours, containing elements of embryonal carcinoma (10). In 3 of these 16 tumours AFP was localised definitely in the embryonal carcinoma cells; in an additional 6, AFP was also detected but it could not be decided whether AFP was present in embryonal carcinoma cells or in EST cells during early differentiation. In 2 of 7 immature teratomas, AFP was shown to be present in cylindric epithelia. All seminomas (4) studied were AFP-negative. These results show that AFP, which occurs regularly in EST, may also be present in embryonal carcinomas as well as in immature teratomas. Thus, it seems that the immunohistochemical demonstration of AFP by the PAP technique is a suitable method of identifying late stages of embryonal carcinoma or early stages of endodermal sinus tumour during the process of differentiation.

This report describes the modulation of malignancy through coinoculation in vivo of a malignant mouse (A9) cell line, and a phenotypically stable, nontumorigenic hybrid clone derived from it by fusion with a transformed mutant human cell. Thus, the tumorigenicity of the A9 cells was suppressed by the hybrid cells, but not by unrelated normal cells. The hybrid cells also inhibited tumor development of unrelated human cancer cells.
