Cytotoxic agents used for the treatment of breast cancer did not inhibit the growth of uterus following estradiol injection to ovariectomized rats. They had no effect on the estradiol binding ability and on the dissociation constant of estradiol receptor binding of rat uterus. In addition, 1,2:5,6-dianhydrogalactitol, a highly active, proximal alkylating agent, applied in vitro did not alter the relative binding ability of tamoxifen to the cytoplasmic estradiol receptor of rat uterus and human breast cancer tissue. In summary, the present experiments did not reveal any disadvantageous interaction between simultaneously administered hormonal and cytotoxic drugs.

HLA determinations were made at the time of diagnosis among a lot of 141 patients with breast cancer. Values of relative risk as well as frequencies of antigens at the first and the second locus were confronted to data found in a control population. Subdivisions of the neoplastic population according to hormonal status and parity led to the emergence of significant differences: frequency of A28 in menopausal nuUiparous women with breast cancer is 26 vs. 7% in controls (p corrected = 0.038) with relative risk = 6.63 (p 0.001).

91 women with metastatic breast cancer, who received prior CTX-5FU or CTX-5FU-PRD, were treated in 3 consecutive clinical trials with either CTX-5FU-PRD-MTX-VCR, MTX-VCR, or MTX-VCR-PRD in order to elucidate whether the effectiveness of 5 drugs was due only to the newly added drugs (MTX-VCR ± PRD) or whether the previously used drugs (CTX-5FU) were necessary as potentiating agents. There were 17 of 39 responders (43%) in the 5-drug group, 3 of 25 (12%) in the MTX-VCR group and 3 of 27 (11%) in the MTX-VCR-PRD group. The survival in the group treated with 5 drugs was significantly longer. Our results show that CTX-5FU-PRD-MTX-VCR was significantly more effective than MTX-VCR or MTX-VCR-PRD used alone after the patient had already had CTX-5FU ± PRD. Thus, the previously used drugs (CTX-5FU) seemed to improve the response to the new agents (MTX-VCR ± PRD) by a potentiating action in the subsequent combination. In 16 patients with cervical carcinoma (stages II and III) tumor-specific and general immune reactivity was investigated. Cancer patients’ skin reactivity was different from that of controls insofar as there was a decreased response to streptokinase-streptodornase as well as to DNCB. Absence of DNCB sensitization was mainly in stage III patients. Cell-mediated immunity to autologous and/or allogeneic tumor-associated antigens (TAA) was demonstrated in vivo and in vitro. 3 of 8 patients had positive skin reactions to autologous TAA and 8 of 14 reacted to allogeneic TAA. In the lymphocyte migration inhibition test, 8 of 14 patients reacted to autologous tumor membrane preparations and 3 of 10 to allogeneic pooled preparations. In addition, there was some indication of cross-reaction with antigens from a human squamous cell carcinoma of the lung. There was no reaction in vitro to normal cervical tissue and no association between tumor-specific reactions and herpes simplex virus type II antibodies could be demonstrated. To determine the degree of correlation among the various in vivo and in vitro assays that could be used to assess immunocompetence in bronchogenic carcinoma, the response to common recall skin antigens, primary sensitization to DNCB and lymphocyte function tests based on blastogenic response to mitogens and in mixed lymphocyte culture (MLC) were tested in 48 patients with bronchogenic carcinoma. The values were compared to responses in 94 age-matched healthy control subjects. There were impaired skin test reactions among squamous cell carcinoma patients, but little impairment of their lymphocyte blastogenesis reactions. Anaplastic carcinoma patients had notable defects in lymphocyte function tests but less impairment of the skin test reactions. These data suggest that the mitogen concanavalin A, phytohemagglutinin, and the MLC are more useful screening assays of in vitro immunocompetence than are the other commonly used mitogens.

To determine whether impaired cellular immune responsiveness in lung cancer patients is related to a reduction in the number of T lymphocytes, circulating levels of T-rosette-forming lymphocytes were measured in 48 cases with primary lung cancer and compared to levels in 94 age-matched healthy subjects. The results show that T lymphocyte levels are significantly reduced in lung cancer patients. A possible consequence of reduced numbers of circulating T lymphocytes would be a diminution in tumor-inhibiting (T-cell) influences and a corresponding relative enhancement of blocking (B-cell) influences, a circumstance which would favor tumor proliferation.

Colony-forming cells (CFC) and colony-stimulating activity (CSA) in peripheral blood cells have been studies before and repeatedly during treatment of 32 patients with acute myelogenous leukemia. WBC obtained after Isopaque-dextran separation were cultured in vitro by a double-layer agar technique. Before treatment 19 patients out of 32 had CSA and all had CFC; both CSA and CFC were found in 19 patients. In follow-up studies during treatment, CSA was mainly unaffected during the leukopenic phase, while CFC were suppressed. No CFC were found at WBC counts below 1,000/mm3. This seems to imply that CFC are more sensitive to chemotherapy than colony-stimulating cells. 14 patients entered remission; all of them had CSA. 16 out of the 18 nonresponders lacked one or both types of cells. The presence of CSA and CFC in peripheral blood therefore appears to be a sign of favorable prognosis, while the absence of CSA and/or CFC implies lack of response to treatment. The conclusion from this study is that the presence of both CFC and CSA in peripheral WBC is a sign of a good prognosis. Absence of CFC and/or CSA in the initial sample indicates that the patient is unlikely to respond to conventional therapy with cytotoxic drugs. The presence or absence of CSA alone has a higher prognostic significance than the presence or absence of CFC.

6 cases of endodermal sinus tumor of the ovary are presented. In 4 patients pure endodermal sinus tumor was found microscopically. 1 patient had endodermal sinus tumor in one ovary and gonadoblastoma in the contralateral one. In another case endodermal sinus tumor was accompanied by an embryonal teratoma. Histologically, the tumor had characteristic features with meshwork of spaces and channels lined by embryonal cells, glomerulus-like structures known as Schiller-Duval bodies, solid aggregates of epithelial cells, hyaline basement membranes and round, PAS-positive small globules found both intra- and extracellulary. In 1 patient the elevated serum α-fetoprotein was stated. All patients were treated surgically with adjunctive radiation and/or with chemical agents. None of them were cured. The median duration of survival amounted to 8.5 months. Discussing the value of the more recent approach to diagnostic and therapeutic methods found in the literature, it must be emphasized that the demonstration of elevated serum α-fetoprotein in patient with that tumor lend not only further support to its yolk sac origin but also might be useful to monitor response to the therapy applied. It is also of prognostic significance by indicating the presence of residual or recurrent disease, even in its subclinical stage. Combined postoperative irradiation and triple chemotherapy according to the VAC regimen of patients can prevent recurrence and in some cases even may cause permanent remission of the neoplas
