42 cases of male breast cancer were studied. The disease is diagnosed in males at a later stage than in females. The prognosis of this disease in males is more favorable than in females: in stage I there is an 83% 5-year survival, while in stage II there is 62% 5-year survival. Male breast cancer shows a high hormone dependency: 80% of patients are improved following orchiectomy and 64% following stilbestrol therapy. Data is reported regarding age, family status, community origin, histology, treatment and survival.

Cytosine arabinoside was administered to 449 patients with a variety of solid tumors. The phase I study included doses of 0.8–3.5 mg/kg × 10 days, and a small number received 5 mg/kg daily for the first three days of each week. The phase II dosage of 3.5 mg/lg × 10 days was given to 225 evaluable patients. Toxicity was seen at all dosage levels. Anemia occurred in 24–57%, leukopenia in 34–70%, and thrombocytopenia in 8–40% with a tendency for increasing incidence with dosage. Striking rebound of platelet counts after the nadir was seen in 22–37% with counts of over 1,000,000 in 2.7%. Responses occurred in a small number of patients (32/377 evaluable cases). They occurred at all dosage levels clustering in dosages of 2–3.5 mg/kg. The most responsive tumors were breast (8/52), gastrointestinal adenocarcinoma (14/124), and soft tissue and bone sarcomas (3/26). The responses were of short duration and rarely of clinical benefit considering the level of toxicity. Studies performed on 5 cancer patients (3 bronchogenie carcinomas, 1 pancreatic carcinoma, 1 female choriocarcinoma) resulted in evidence that (1) some malignant tumors may contain a small amount of antidiuretic activity identical or very similar to posterior pituitary antidiuretic hormone (ADH) even in the absence of the syndrome of inappropriate ADH secretion; (2) the zone of active tumor growth contained most (50–100%) of the antidiuretic activity; (3) para-endocrine-active tumor cells can release ADH into Krebs-Ringer incubation solution, and they may be capable of synthesizing a substance identical or very similar to posterior pituitary ADH; (4) ADH levels measured in blood plasma and urine reflect the overall ADH production rate in tumor tissu

The cellular response and serum blocking factors of 70 cancer patients were assessed by micro-MLC and PHA-stimulation techniques. Serum level of carcinoembryonic antigen (CEA) was determined by radioimmunoassay, and an attempt was made to correlate the concentration of CEA with the inhibitory effect of each serum on the blastogenic response of lymphocytes from cancer and healthy subjects. A direct correlation was found between the serum inhibitory activity, the CEA level and the extent of the disease. However, CEA negative cancer sera also inhibited transformation of autologous and normal homologous lymphocytes. This indicates that circulating CEA does not account for serum blocking activity.

A 22-year-old Chinese man presented in pulmonary crisis with massive bilateral pulmonary metastases from choriocarcinoma. Because of a diagnostic error, he was initially treated with medroxyprogesterone and responded with a marked lowering of serum luteinizing hormone and lysis of his fever – suggesting that progestins may be useful in the treatment of choriocarcinoma. When his true diagnosis was established, he was treated with conventional poly chemotherapy. He had persistant elevations of serum estrogen and testosterone levels throughout his course, a feature not previously reported in choriocarcinoma in males. Although he was given very large doses of chemotherapy, and achieved a partial remission, he suffered no bone marrow toxicity. It is suggested that the elevated levels of circulating testosterone protected him from bone marrow toxicity. Androgens may be an important adjuvant in the chemotherapy of cancer. Elevated titres of LH (but not sex steroids) were found in his cerebral spinal fluid when he developed cerebral metastases, suggesting that the malignant trophoblast secretes chorionic gonadotropin (but not sex steroids) which, in turn, stimulates the secretion of sex steroids in target glands. A patient with mixed cell Hodgkin’s disease, who 13 years later presented with lymphoblastic lymphosarcoma is described. Whether this association represents a transformation of one form of lymphoreticular neoplasm to another or a new neoplastic process cannot be determined. The newer therapeutic modalities for lymphoma produce prolonged survival. It is, therefore, imperative that patients in remission, who exhibit a recurrence of adenopathy, be re-biopsied, since the optimum treatment differs for each of the lymphoma

43 patients with gastrointestinal carcinoma were treated with a sequential combination of BCNU and 5-FU. There were no responses among 25 patients previously exposed to chemotherapeutic agents, including 11 patients without prior exposure to the study agents. Among the 15 previously untreated patients there were responses in 2/4 gastric carcinomas and 1/5 colon carcinomas. It is suggested that the data from this and other reports indicate the need for further trials of the combination in gastric and pancreatic carcinomas.

Evidence is outlined associating proteolytic events with initiation of lymphocyte blast transformation (LBT) in both antigen-specific and nonspecific mitogen-induced stimulation. It is proposed that lymphocytes normally express the hinge region of the IgG molecule on their surface and that this peptide (pro – thr – cys – pro – pro – pro) approximately constitutes a trigger peptide or detendomer that, by virtue of its proteolytic cleavage, stimulates the cell to LBT and mitosis, the trigger becoming inoperative when NAGA is linked to the threonine. The concept is extended to the malignant cell, which would express a detendomer similar in amino acid sequence and function to the lymphocyte blast detendomer, and would be triggered in a similar way.
