42 consecutive patients with head and neck squamous cell carcinoma were entered into a multi-agent chemotherapy protocol including: bleomycin and Cis-platinum (in continuous infusion), sequenced with methotrexate and Vinblastine (by bolus infusion). Hydroxyurea was substituted for bleomycin when the accumulative dose of bleomycin reached 400 units. 9 of 11 patients with untreated disease responded to chemotherapy, with 36.4% showing complete response following the second cycle. After completion of all therapy, complete response had occurred in 10 patients and partial response in 1. At a median follow-up of 1 year, 9 patients are disease-free, and the median duration of response has not yet been reached. For recurrent disease, the overall regression rate is 45.1% (14/31). The median duration of response is 6.5 months, with responding patients showing a median survival of 8.7 months and non-responders, 5.4 months. The data for response and survival are comparable to previous studies and suggest that toxic manifestations may be mitigated by careful attention to nutrition and production of an anabolic state prior to therapy.

Epidoxorubicin (EDX) was administered to a group of 26 patients with acute leukaemia or lymphoma, replacing either daunorubicin or doxorubicin (DXR) as a component of multiple-chemotherapy regimens. In 10 patients with acute non-lymphocytic leukaemia treated at presentation, 5 complete and 4 partial remissions were obtained. All patients with lymphoma of different types responded at least partially: in a group of 14 cases, 8 in relapse and 6 at presentation, there were 10 complete and 4 partial remissions. It is of interest that such results were obtained in patients of high median age with dosages of EDX much lower than those employed for DXR.

Recently, several investigators have demonstrated the presence of triphenylethylene antiestrogen binding sites in the cytoplasm of many tissues, which specifically bind to radioactive tamoxifen with high affinity. Although mammary gland is one of the principal target organs for antiestrogen action, the characterization of antiestrogen binding in mammary tissues has not been reported. We have studied the antiestrogen binding properties of [3H]tamoxifen in the mammary glands of virgin, pregnant and lactating rats as well as in the N-methyl-N-nitrosourea-induced mammary tumors. Tritium labeled tamoxifen bound specifically and with high affinity (Kd = 10––9M) to components present in 25,000 g supernatant. Unlabeled estradiol or DES did not compete for these sites, whereas unlabeled tamoxifen showed competitive inhibition. The mammary glands contained threefold higher levels of cytoplasmic binding sites as compared to the mammary tumors. In 116 subjects, serum ribonuclease (RNase) and ferritin were determined in order to evaluate whether their combined evaluation might improve the diagnostic accuracy of each test. Significantly higher levels were found in pancreatic cancer patients both for RNase and ferritin than in control subjects and chronic pancreatitis. Sensitivity and specificity in diagnosing pancreatic cancer were 86% and 46%, respectively for RNase; 76% and 65% for ferritin. One of the two tests was pathological in 100% of pancreatic cancer, with a specificity of 29.9%; both were pathological in 62.1%, with a specificity of 82.1%. The results emphasize the limits of the combined assessment of pancreatic cancer markers.

In a Balkan nephropathy endemic area, 5 siblings developed haematuria over a period of 10 years. 4 of them were operated on and pathohystological examination revealed papillary transitional cell tumours of the renal pelvis and/or ureters. Both these tumours and the Balkan nephropathy affected only 1 generation of the family members. This fact, as well as the other relevant evidence, indicated that environmental factors played an important role in the process of carcinogenesis.

A 67-year-old patient with breast carcinoma on treatment with aminoglutethimide (AG) developed profound leukopenia and sepsis. WBC count recovered within 1 week and the drug was renewed without further complications. The hematological toxicity of AG is reviewed. Detailed descriptions of this side effect of AG are usually lacking in most published series. It appears though that blood dyscrasia due to AG occurs rarely and is promptly reversible upon discontinuation of the drug, but renewal of its administration is not always possible.

The natural resistance of human chronic myeloid leukemia cells to adriamycin (ADR) was overcome by using the cell membrane modulator verapamil (VRP). The studies were done in vitro. [3H] thymidine incorporation inhibition was used as a measure of cytotoxicity. The results clearly show an increase in the effectiveness of ADR when used along with VRP, which may be due to the changes in the cell membrane brought about by VRP.

The activity inducing PMN aggregation, the chemotactic activity, and the activity augmenting PMN adherence were estimated in plasma of 40 untreated patients with Hodgkin’s disease. The first two activities were evident in all patients with advanced clinical stages (III and IV). It seems to be the result of disseminated intravascular complement activation due to the circulating immune complexes. The plasma activity augmenting PMN adherence was similarly noticed, except for 1, in patients with advanced stages of the disease, but only when the general symptoms were present. A murine bladder tumor model was evaluated for tumor growth retardation while receiving combination chemotherapy, either 40 mg/m2 methotrexate and 35 mg/m2 cis-platinum or 500 mg/m2 Cytoxan and 35 mg/m2 cis-platinum at 3-week intervals. Tumor growth was observed in the control and treated groups. A significant decrease (p < 0.001) in tumor growth and lung metastasis was noted in the Cytoxan and cis-platinum group. Although the 40 mg/m2 group treated with methotrexate and 35 mg/m2 0.1), and lung metastasis was greater than in the control group. Cis-platinum with Cytoxan was clearly effective in retarding tumor growth and metastatic spread.
