ABSTRACTThe effects of chilli on gastrointestinal transit (gastric emptying, orocaecal transit, whole gut transit) were evaluated in eight healthy volunteers. In each subject, gastrointestinal transit of a standard test meal was measured on two separate days. On one of these occasions, 20 g of chilli powder was added to the meal. Gastric emptying was quantified with a radioisotopic technique, orocaecal transit by measurement of breath hydrogen concentrations and whole gut transit by counting the number of radio‐opaque markers in the stool. The rate of gastric emptying was slower (P<0.05) and whole gut transit was faster (P<0.02) after the meal containing chilli, compared with the other meal. ABSTRACTThis study examined Na+/H+ion exchange of caecal and colonic mucus. Two distinct Na+/H+ion exchange populations of caecal mucus were identified, one group being a cation exchanger and the other an anion exchanger, suggesting that caecal mucus has ampholyte properties responsive to changing physical and chemical properties of luminal contents. ABSTRACTOne hundred and forty‐two female patients consulting a prosthodontic clinic for masticatory disturbances and suffering from mandibular ridge atrophy were systematically interrogated for the presence of digestive symptoms. Eighty‐five subjects (60% of the studied population) reported current digestive complaints; 32 had abdominal pain (burning sensation, bloating or cramps), 12 presented stool transit alteration (constipation or diarrhoea) and 41 reported both abdominal pain and stool transit abnormalities. A prospective evaluation of the digestive symptoms was obtained following surgical reconstruction of the atrophic mandibular ridge and insertion of functional dental prostheses to correct masticatory dysfunction. One year after jaw surgery, 62 of 73 patients (85%) initially complaining of abdominal pain reported symptomatic improvement of their condition, while a marked amelioration in stool habits was noted in 34 of 53 patients (64%) initially suffering intestinal transit alteration. ABSTRACTA 57 year old man with auto‐immune chronic active hepatitis, regularly treated with immunosuppressive therapy, had hepatocellular carcinoma (HCC) 10 years after diagnosis of the hepatitis. Assays of the hepatitis C virus antibodies against capsid and non‐structural proteins revealed seronegativity in serial serum samples of this patient stored in the previous 10 years during follow up. The seronegative hepatitis C antibodies excluded hepatitis C virus as the cause of the HCC. The occurrence of HCC in this case suggests the necessity of surveillance for early detection of liver cancer in patients with auto‐immune chronic active hepatitis undergoing long‐term immunosuppressive

ABSTRACTTo determine whether upper gastrointestinal endoscopy poses a risk to patients with arthritis of the neck, 486 members of the Gastroenterology Society of Australia were surveyed by postal questionnaire. Replies were obtained from 323 members. More than 1.5 million endoscopies were covered and 11 respondents had experienced major complications related to neck arthritis. With one exception these were perforations of the upper gastrointestinal tract. There were no reports of serious neurological sequelae. Deaths resulted in two cases. We conclude that neck arthritis rarely causes complications from endoscopy.
