摘要:
PurposeThe purpose of this study was to develop and evaluate an experimental body porcelain tooth shade‐guide system based on in‐vivo color measurements. This system was tested against two widely used commercial body shade porcelains.Materials and MethodsA body porcelain shade guide consisting of 16 tabs was fabricated from available opaques, body, and incisal porcelains and modified with metallic oxide tints. The target Hue, Value and Chroma of these tabs were based on middle‐site in‐vivo tooth color measurements reported by Goodkind and Schwabacher in 1987. Two control groups of body porcelains were fabricated from Ceramco and Bioform porcelains. A new Dental Shade Comparator was used in a clinical test of these tabs in visually matching middle sites of 300 maxillary anterior teeth of 103 subjects.ResultsThe demographic trends in Hue, Value and Chroma were in agreement with those found in the previous study. Four of the 16 experimental body shade tabs were frequently chosen compared with 13 of 16 Ceramco and 3 of 25 Bioform body shades. Intraexaminer and interexaminer reliability tests showed good agreement using the new Dental Shade Comparator.ConclusionsThe 20 shade tabs that were most frequently selected as a 1st or 2nd choice ranged from 8.25YR to 1.80Y in Hue, 6.45/to 8.00/in Value and/2.05 to/4.15 in Chroma. The new Dental Shade Comparator is a useful tool in the task of shade matching. The current Ceramco II body shades contain many useful porcelains that should be augmented with higher value and with redder, darker

摘要:
PurposeThis study characterizes the distribution of the mesiodistal widths of maxillary anterior natural teeth for comparison with the widths of the most commonly used artificial denture teeth.Materials and MethodsThe width of the six maxillary anterior teeth was measured on a curve defined by the facial surfaces of the teeth. Four hundred fifty‐one casts of completed orthodontic patients (154 men and 297 women) were measured separately by gender. The width of the maxillary anterior natural teeth was compared to that of artificial denture teeth (Bioblend, Dentsply International, York, PA).ResultsThe mean ± standard deviation (SD) width of the six anterior teeth for men and women (55.6 ± 3.1 mm and 53.6 ± 3.0 mm, respectively) differed significantly (P<.05). The mode (most frequent size) was 55 mm for both men and women; however, 40% of the women and 67% of men had anterior teeth that measured 55 mm or wider. In contrast, the mean width of the most widely distributed artificial maxillary denture teeth (Bioblend) is 50.1 mm, with a range of 44 to 58 mm. Of the 42 molds available, only 5 have a width of 55 mm or greater.ConclusionsThese findings suggest that one reason for the inappropriate selection of comparatively small maxillary denture teeth is the lack of physiologically sized tooth m

摘要:
PurposeThe oral effects of diabetes in dentate patients have been studied extensively, but little clinical data has been collected with regards to edentulous patients who have diabetes. The purpose of this study was to investigate whether there are differences between diabetic and nondiabetic patients in initial patient tolerance and tissue response to complete dentures.Materials and MethodsComplete dentures were constructed for 62 diabetic and 56 nondiabetic patients. Treatment followed accepted prosthodontic principles recommended for diabetic patients. Twenty‐four hour and 1 week post‐placement appointments consisted of a patient questionnaire, intraoral examinations and indicated adjustments. The variables studied were the differences between responses to the questionnaire and the frequency of type, location, and size of clinically visible areas of mucosal irritation reported on standardized tissue evaluation forms.ResultsAnalysis of the responses to the questionnaire indicated diabetic patients reported xerostomia more often than nondiabetics. There were no statistically significant differences in initial tissue response to new complete dentures between the diabetic and nondiabetic patients. A statistically significant difference in tissue response was reported within the diabetic group, significantly fewer diabetic patients exhibited oral irritations at the second examination than at the first examination.ConclusionsThe results of the present study indicate that when prosthodontic principles recommended for diabetic complete denture patients are followed, there are no clinically significant differences between diabetic and nondiabetic patients in initial patient tolerance and tissue response to complete dentu

摘要:
An accurate marginal fit of a dental casting is one of the goals of restorative dentists. Use of specific indirect techniques facilitates fabrication of the most precise castings. These techniques incorporate a series of orderly and logical steps on multiple analogues to eliminate potential errors. Evaluating precisional fit of the casting requires the coordination of psychomotor skills and prescribed materials usage. Evaluation of marginal fit must be accomplished indirectly. It is easier and more precise to appraise marginal accuracy on an analogue than intraorally. Training increases one's ability to evaluate margins. Currently there is an increased demand for casting accuracy to restore endosseous implants. The immobility of dental implants imposes a high order of precision on both indirect techniques and indirect evaluation to achieve success. The inability to achieve this level of precision can result in implant and/or prosthesis failure.

摘要:
A method for occlusal reshaping is described using a step‐by‐step analysis starting from the articulation of models to the completion of the clinical procedures. Several newly introduced definitions are provided to help the reader understand the procedures. This article is divided into three parts: (1) the preliminary procedures, (2) the occlusal reshaping of the study casts, and (3) the clinical occlusal reshaping. The advantages of this particular method include establishment of anterior guidance before initiating posterior reshaping procedures, maintenance of maximum cusp height and fossae depth, the placement of forces along the mid vertical axis of the teeth, and the establishment of simultaneous even contact of the posterior teeth, while providing the desirable anterior guida

摘要:
A literature review of the selection of the maxillary anterior teeth is presented. Although several different qualitative guidelines are cited, the best method for the selection of the maxillary anterior teeth is the use of pre‐extraction records or old photographs. Numerous qualitative and quantitative guidelines are cited in the literature for the proper placement of the maxillary anterior teeth. Through careful and proper construction of the maxillary wax occlusal rim, these factors may be recorded and relayed to the dental laboratory technicia
