摘要:
Epilepsy is a chronic medical condition that affects many children The impact of epilepsy on children's psychosocial life and development are surveyed, and some of the problems faced by children and adolescents with epilepsy are identified The importance of experiencing success, developing self-respect, and learning to be independent are stressed, since these issues are important for adult success and happiness Health care workers need to know about the totality of health care needs of children with epilepsy, including medical and psychosocial factors, so they can act as advocates to help remove the barriers these children face.

In recent years, pediatric hospitals have made many adjustments in their routine regulations in order to promote the emotional health of child patients and their families In response to recommendations of such organizations as the American Academy of Pediatrics and the Association for the Care of Children's Health, many institutions are liberalizing their visitation policies to include siblings of hospitalized children In order to discover answers to questions about what is being done regarding this issue, the authors undertook an informal mail survey to explore the current situation primarily in the United States The purposes of this study were to conduct surveys of (a) state health departments to determine the existence of state regulations regarding sibling visitation, (b) pediatric hospitals to determine specific visitation policies, and (c) pediatricians and epidemiologists closely associated with hospitalized children to determine opinions regarding risks and benefits associated with sibling visitation.

Twenty-six boys whose height was a minimum of 2 SD below normal because of either constitutional delay of growth and adolescence or growth hormone deficiency were evaluated to determine per sonality and behavior functioning Other researchers, using unstructured personality assessment techniques, have reported etiology-related behavior differences between these groups of children with short stature Use of an objective, empirically derived, self-report measure and standardized behavior checklist completed by parents did not reveal etiology-specific behavior patterns between groups Age appeared to be a more important factor related to personality adjustment Older boys exhibited a more conforming and less aggressive response style than younger boys Short children, regardless of age, were rated by parents as being more withdrawn than age-matched boys in the normative data on these measures.

This study utilized an inductive approach to identify sources of stress experienced by parents when a child is admitted to a pediatric intensive care unit Based on a thorough rewew of the literature, clinical observations, and interviews with parents of children recently discharged from a pediatric intensive care unit, 79 items defining eight dimensions of pediatric intensive care stressors were identified Suggestions are made for intervention strategies that can be utilized in an attempt to minimize parental stress.

Parents' level of stress as measured by the Parenting Stress Index is examined in relation to the utilization of pediatric medical services for children between the ages of 1 and 48 months The six categories of medical services considered were Well Visits, Sick Visits, Infections, Diarrhea and Vomiting, Traumatic Injuries, and Other Medical Services For the first 18 months of age no significant mean difference between stress groups and medical utilization was found Significant correlations and mean differences were found for the 19- to 24- and the 25- to 48-month age groups in relation to the traumatic injury criterion Moderate parenting stress is interpreted as potentially adaptive in relation to reducing traumatic injuries.

In an attempt to evaluate the services provided in a children's chronic care hospital, the authors administered a 24-item questionnaire to 85 parents Thirty-four parents (40%) responded with their opinions and suggestions The paper outlines the questionnaire, the details of the parents' responses, and the action taken by the hospital The use of a parent questionnaire as a method of introducing changes in the environment of a children's hospital is discussed.

Cystic fibrosis is one of the most complex and progressive chronic illnesses faced by pediatric care practitioners, both physiologically and psychologically Improved medical management has resulted in a longer life span for those with the disease At the same time, such improvements have presented patients with the new challenge of facing adolescent developmental tasks as well as the disease's effects on them The author looks at the impact of cystic fibrosis on various adolescent developmental tasks and briefly discusses approaches that the health care team can adopt to facilitate the patient's development.

This brief report proposes the need for an Ambulatory Child Life Specialist to be added to the staff of a pediatric office The role of such a specialist would include counseling about normal growth and development problems, pre-procedure teaching for scheduled ambulatory tests, creating a supportive atmosphere in waiting areas, and coordinating education for children with chronic illness.
