Examining 61 youths diagnosed with sickle cell syndromes, this study focused on both protective and risk factors of adaptive behavioral functioning in these children. Demographic characteristics (age, sex, intelligence, and socioeconomic status), severity of disease (number of days hospitalized, emergency room visits, hemoglobin, and school days missed), behavioral adjustment, and family functioning were examined to determine their relation to adaptive behavioral competencies. Age, sex, and intellectual functioning were found to be efficient predictors of the majority of adaptive behavioral domains, with higher adaptive functioning being associated with younger children, girls, and greater intellectual functioning. Adjustment difficulties characterized by internalizing symptoms were found to predict communication and global adaptive functioning. Within the context of a risk-resistance adaptation model, the present data were interpreted to support the use of secondary prevention efforts for children with sickle cell syndrome who may be designated at high risk for developmental delays.

We made a preliminary study of the psychological adjustment of children and adolescents with neurofibromatosis (NF), or so-called Elephant Man's Disease. Twenty-four participants, both male and female aged 8 to 18 years, were identified from genetics clinical records; interviewed by telephone; and asked to complete measures of self-concept, adjustment, social support, and perceived NF symptom severity. Data from the 17 families who agreed to participate and returned usable forms yielded mixed support for hypothesized relations among NF severity, social support, and measures of adjustment, with source of report being a key factor. Implications of these preliminary findings for health care professionals working with NF patients are discussed.

Children with chronic or intractable epilepsy are at higher risk for medical and social difficulties. A multidisciplinary clinic was established to improve medical management and address psychosocial issues. The purposes of the present study were to describe the multidisciplinary clinic, retrospectively examine the referral patterns for children served by the clinic, and analyze factors significantly predictive of parental satisfaction with the multidisciplinary approach. The Epilepsy Clinic Satisfaction Questionnaire was sent to all patients seen in the clinic over a 2-year period. Outcome findings were based on 136 respondents. A stepwise regression analysis indicated that the best predictor of parental rating of clinic quality was the amount of information given concerning the diagnosis and treatment of epilepsy. Staff attitude was also significantly related to parental satisfaction.

In this study, we delineated the types of psychosocial stressors experienced by mothers of children with chronic physical conditions. One hundred nineteen mothers, each having a child between 2 and 18 years of age with a motor or sensory disability, were asked on three occasions to describe the most bothersome thing that had happened to them related to their child's disability. A content analysis was then performed on the 246 independent responses to this request. On this basis, four general domains of concern were delineated: medical and legal concerns, concerns for the child, concerns for the family, and concerns for the self. Each domain can be further divided into three to five more specific areas of concern. None of the demographic variables evaluated were found to associate with the type of concern reported. However, specifics of the child's condition were found to be related to the types of problems described. Mothers who reported problems in the area of concerns for the self also reported poorer physical health. However, neither maternal mental health nor maternal physical health were found to be related to other types of concern reported by the mother.

In this study, I examined parental expectations of a child with spina bifida and the child's self-concept, and I explored the relation between expectations and self-concept. The Predictive Adaptive Ability Questionnaire and the Piers-Harris Self-Concept Scale were used to measure parental expectations and the child's self-concept in 30 able-bodied and 30 spina bifida parent-child dyads. Expectations were significantly different for the two groups; however, self-concept scores were not different, with no significant relation between the two. The results suggest that parents have different expectations for a child with a disability, and those expectations primarily reflect physical limitations.
