Thirty-one mothers of infants with sickle cell anemia (n = 14) or sickle cell trait (n = 17) were interviewed to investigate the psychological stress they experienced and the coping strategies they used when first learning their child's diagnosis and when presently caring for their child. All children were diagnosed with sickle cell conditions through newborn screening. Mothers of children with sickle cell disease most often reported the expectation of pain for their child as a stressor, whereas mothers of children with sickle cell trait most often reported uncertainty of the diagnosis as a stressor. Although sickle cell trait is not a life-threatening condition, mothers of these infants reported a great deal of stress in regard to the health of their children and to their own ability to manage such stress. Mothers in both groups reported using Positive Reappraisal, Seeking Social Support, Self-Control, and Planful Problem Solving most frequently to cope with present stressors. The results also have implications for the use of newborn screening.

We compared the knowledge of sickle cell disease, child and parent coping strategies, and parent discipline methods/knowledge in three groups of children and their parents. The groups were composed of: (a) children with sickle cell anemia who experienced disease-related significant lifestyle disruption, (b) children with sickle cell anemia who experienced disease-related minimal lifestyle disruption, and (c) non-ill Black children. The groups were matched for age, sex, and ethnicity. No differences were found among the groups on measures of child coping. Children with sickle cell anemia, regardless of level of disruption, reported more knowledge about sickle cell disease than did healthy peers. Parents of children in the significant lifestyle disruption groups scored lower on the Engagement, Emotional Engagement, and Problem-Focused Disengagement, and higher on the Medical Coping scales of the Coping Strategies Inventory than parents of healthy children and children in the minimal lifestyle disruption group. Surprisingly, the groups did not differ in discipline knowledge; however, parents of children in the minimal lifestyle disruption group reported significantly greater use of effective discipline methods than parents of healthy children. Further research on the relations among coping, discipline, and lifestyle disruption due to SCA in children is suggested.

Pediatric liver transplantation is becoming increasingly common, but specific information regarding the adaptation of the families of these children, particularly of siblings, is lacking. We examined 64 siblings from 45 families of children and adolescents evaluated for liver transplantation using the Child Behavior Checklist and the Impact-on-Family Scale. As a group, the sibling subjects did not show deficits on measures of social competence or behavioral maladjustment. However, 15 siblings showed behavioral maladjustment. These children were not distinguishable from the pool of siblings based on sex, birth order, or age spacing relative to their sibling (the patient), socioeconomic status, or duration of the patient's illness. However, these behaviorally maladjusted children came from families in which the mothers reported significantly greater personal strain than did the other parents in the study. Suggestions are made for interventions to reduce stress in families facing liver transplantation.

Programs that assist parents and children with coping during hospitalization may minimize the emotional trauma of hospitalization and improve both child and family well-being. The purpose of this study was to determine the effect of parental visitation in the Post-Anesthesia Care Unit (PACU) on pre- and postoperative anxiety levels of pediatric patients and parents. Sixty-eight patient-parent pairs were enrolled in the study: 20 before implementing parental visitation and 48 following implementation. Although no differences were found in children's anxiety levels pre- to postoperatively, the anxiety levels of parents who visited their children in the PACU were significantly lower postoperatively.

Thirty-four nurses who were employed more than 20 hr per week in a neonatal intensive care unit (NICU) at a midwestern teaching hospital participated in a survey on discharge teaching. The survey consisted of 43 topics on which parents may or may not receive instruction prior to their baby's discharge from the NICU. For each topic, nurses were asked how important the topic is for parents to learn about prior to discharge, whether parents in the NICU are currently receiving instruction on the topic, and who among the NICU staff is responsible for instruction on the topic. Data obtained from this survey were compared to data obtained from a similar survey conducted 18 months previously with parents in the same NICU. Nurses rated the majority of topics in the survey as being very important for parents to learn about prior to their baby's discharge. For many of the items, a significant discrepancy existed between the percentage of nurses reporting the topics that were routinely taught and the percentage of parents who actually recalled learning about the topics. Suggestions on ways to improve the effectiveness of discharge teaching are provided.
