This paper analyzes the state of knowledge in four central areas of pediatric pain consequences, assessment childrens experiences, and management The authors point out the general lack of research, common beliefs of health providers, and contradictions and uncertainties in the literature Difficulties in identifying and measuring childrens pain are cited as the major barriers to effective assessment and management and to a firm scientific base Recognition of the general lack of knowledge, more empirical focusing, rejection of unproven assumptions, and more attention to clinical reasoning are recommended as starting points for reducing the uncertainty about pain in the care of hospitalized children.

Care determines cure and, therefore, the mode of care dictates the outcome The medical needs do not determine care, but rather define the reality within which the child has to live Where that care is given to the child should be a haven, just as home ought to be a secure place to be oneself The child participates in the therapeutic community as much as the child participates in the family Therefore, the treatment environment of the child should be designed to foster caring toward cure The care that is given will determine the truly cured child The children must be allowed to be themselves so that they can be cared for and thereby their cure assured To accomplish that, medical care must be incorporated into a normalizing, structured therapeutic community wherein the child can exercise autonomy and the care givers, compassion.

An analysis of play interviews with 10 chronically ill 2- to 6-year-old hospitalized children revealed that 9 of the 10 children were able to use clinical equipment to express their concerns about intrusion Four types of intrusive play activities were identified from the content analysis rejections, medications, temperature-taking and tube-feeding Play about injections accounted for 50% of the childrens play However, the fact that 40% of play represented medications and temperature-taking suggests that even "routine" procedures may be a source of more concern to children than we, as caretakers sometimes realize The results also revealed the individuality with which each child expressed his or her intrusive concerns Discussion includes recommendations for assisting health professionals to help children master their concerns about intrusion.

Environmental conditions and waiting behaviors of 625 children and their families were recorded in the waiting room and suture hall of a childrens hospital and a general hospital Randomized intervention and control days at the childrens hospital resulted in children and their families being assigned to an experimental group receiving child life intervention or to a control group who did not receive child life intervention A second control group at the general hospital did not receive child life intervention The average waiting time was 1 5 hr Sigmficant differences were observed in (a) noise level between the children's hospital and general hospital waiting areas, (b) waiting behaviors of children who received child life intervention and those who did not, and (c) parent-child interaction among the three groups Suggestions for further study regarding the optimal use of waiting time are included.

Automobile accidents are one of the leading causes of death and injury in children According to safety officials, the consistent use of safety restraints could prevent over 60% of these tragedies Yet, the majority of parents continue to allow their children to ride unrestrained This study of 102 parents of preschool children suggests that the more barriers parents see to the use of restraints the less likely they are to use them Suggestions are made for minimizing these barriers, which should enable more children to ride safely.
