The effects of phenoxymethylpenicillin on the bacterial flora of the vagina were investigated in 6 women who fulfilled the criteria of ‘normality’: predominance of lactobacilli, and absence of Enterobacteriaceae and fungi. Fourteen healthy women were enrolled and followed longitudinally; 8 of these were subsequently excluded because of divergence from normality. Following penicillin treatment, Gram-negative rods appeared in 4 of the remaining 6 women, and in 1 subject heavy growth of Candida albicans resulted. In 5 of the women there was no change in the lactobacillus count and a rise in pH was found in 4 subjects. In 1 woman, lactobacilli disappeared completely and were replaced by Escherichia coli. Concomitantly, high concentrations of endotoxins were found in the vaginal fluid. The growth fractions (GF) of 151 mammary carcinomas were determined in situ by Ki-67 immunoperoxidase staining. A mean value of 14.6% Ki-67-positive tumor cells, with a standard deviation of 11.1, was found. Tumors of histological grades 1 and 2 had a mean GF of 13.1 % which was significantly lower (p < 0.05) than that of grade 3 tumors (mean GF 17.3%). The 14 patients in whom distant metastases appeared after primary staging had a significantly higher (p < 0.05) GF (21.3%) than the other patients (13.9%). No significant correlation was found between GF and concentration of hormone receptors, lymph node status, tumor size, or patient’s age. Our results suggest that immunostaining with Ki-67 could be used as an additional tool to detect breast carcinoma patients with a high risk of developing metastases.

An unusual case of a full-term pregnancy in a patient with a uterus didelphys is presented. The main complication was the formation of pyocolpos in a blind hypoplastic hemivagina observed early in gestation. The therapeutic approach and the related problems are discussed.

Uterine leiomyoma associated with ascites and hydrothorax was apparently cured by total hysterectomy. This is believed to be the 15th recorded case of this type of Pseudo-Meigs’ syndrome and the first in a patient with severe curvature of the spine and dorsolumbar scoliosis. The association with ascites and hydrothorax was investigated using 99mTc-labeled macroaggregated albumin.

Two patients with granulosa cell tumors of the ovary, endometrial hyperplasia, elevated serum estradiol (E2) concentrations and depressed FSH levels were studied preoperatively using intravenous administration of FSH, LH and gonadotropin-releasing hormone. In patient 1 serum E2 increased from a baseline of 72.7 to 116.8 pg/ml 60 min after hCG stimulation, with a peak level of 571.4 pg/ml 96 h after hCG administration; there was a rapid postoperative decline to 16 pg/ml. Ovarian venous E2 on the tumor side was 6,979 pg/ml. In patient 2, E2 increased from a baseline of 91 to 449 pg/ml at 20 h after FSH administration. Intraoperative ovarian venous E2 was 9,788 pg/ml. Inhibin, which was elevated in patient 1 prior to stimulation (736 fmol/ml), peaked 96 h after hCG administration. Ovarian venous inhibin concentration in patient 1 was 2,911 fmol/ml. The baseline inhibin concentration in patient 2 was not elevated (249 fmol/ml), but there was an elevation of inhibin in response to FSH administration (757 fmol/ml), but no response to hCG. Ovarian inhibin concentration was not different between the tumor and contralateral sides in patient 2. Both granulosa cell tumors were responsive to exogenous gonadotropin stimulation. Although only one tumor made significant baseline levels of inhibin, the production of inhibin by both tumors was apparently stimulated by FSH.
