Nafarelin, a gonadotropin-releasing hormone agonist, has been shown to be effective in the treatment of endometriosis. The standard dosage is 200 µg bid intranasally. Side effects most commonly include those associated with estrogen deprivation. By assessing estrogen status, the standard dosage can be manipulated to minimize these side effects and increase patient compliance

Clinical observations indicate that there is an association between hyperinsulinemia and hyperandrogenism in women of reproductive age. The effects of insulin on ovarian steroidogenesis were studied in 4-day incubations of minced porcine theca. Insulin alone significantly stimulated dihydrotestosterone (DHT), testosterone (T) and androstenedione (Δ4A) accumulation in the media as compared to vehicle-treated controls. Insulin alone did not stimulate progesterone (P) accumulation. Insulin plus luteinizing hormone (LH) resulted in significantly greater accumulation of DHT, T and Δ4A than LH alone. Insulin plus LH did not stimulate P accumulation as compared to LH alone. Insulin, at high concentrations, may be a regulator of thecal androgen synthesis.

In a group of 316 conizations performed from January 1984 to July 1991, 68 patients (22%) revealed no free margins of cervical intraepithelial neoplasia (CIN). They were divided into two groups with: ‘negative follow-up’ [normal cytology or less than two smears suggesting CIN I (class IIIa)] and ‘positive follow-up’ [at least two smears suggesting CIN I or one smear suggesting CIN II (class IIIb) or worse]. Forty-five patients (66%) fulfilled the criteria of negative follow-up. All patients with CIN I at the cone margin (18) were found in this group. The other 23 patients were in the positive follow-up group. Patients with positive follow-up were significantly younger (p = 0.04). Intervention after incomplete cone biopsy was necessary in approximately 10% (7/68) of cases. Cytology was thereby a reliable indicator for residual disease. Higher grade of CIN involved at the cone margin predicted a higher chance of positive follow-up. Semiquantitative measurement of CIN in the cone specimen showed a severe involvement of CIN II and CIN III in the positive follow-up group. In sum, our study showed that CIN I at the margin of the cone biopsy has no clinical significance. However, CIN II or III at the margin needs a careful cytologic follow-up, and appropriate action should be taken in case of cytologic abnorm

Estrone (E1), a principal estrogen in postmenopausal women, may have profound consequences in the maintenance and progression of hormone-sensitive endometrial carcinoma. This study was designed to investigate the specific binding sites for E1 in the tumor and in the normal endometrium. The binding of [3H]E1 to the cytosolic fraction and KCl-soluble nuclear fraction from 3 endometrial carcinomas showed saturation kinetics with an apparent equilibrium dissociation constant of approximately 37 and 50 nM, respectively. The specific binding site of E1 was also found in 3 normal endometria. However, the binding capacity in the endometrial carcinoma increased to 1.5-fold of that in the normal endometrium. E2 did not affect [3H]E1 binding to any sub-cellular fraction from endometrial carcinoma specimens. These findings demonstrate the presence of specific binding sites for E1 in human endometrial carcinoma. The endometrial carcinoma growth may be mediated, at least in part, by E1 and its binding site interaction.

We report a successful twin pregnancy in a woman with secondary amenorrhea who is genotypically 47, XXX. Cytogenetic markers suggested that they are apparently monozygotic twins.
