Immature teratoma cells obtained from a patient’s ovary were cultured and morphologically characterized in the early culturing stage. The major part of the tumor was composed of neuroglial-like tissues which were surrounded by neuroepithelial-like tissues. The neuroglial-like cells cultured in vitro morphologically differentiated to the epithelioid cells and the morphologic pattern of the culture came to resemble that of the original lesion. Furthermore, some kinds of the differentiated cell grew out from the cell aggregate that grew on the colonies of the neuroglial-like cells. The morphological character of the cultured immature teratoma cells suggests that neuroglial-like cells have totipotency to change to differentiated cells forming a teratoma.

Urodynamic examinations carried out on 45 patients with stress urinary incontinence (SUI) and 17 women without a history of incontinence using simultaneous microtransducer urethrocystometry were examined in order to develop an objective indicator of the severity of the condition. Five urethral pressure profiles (UPP) with stress were recorded, maintaining a constant coughing strength as seen in the bladder pressure rises. The coughing strength was increased stepwise for successive profiles. Zero urethral closure pressure, indicating genuine SUI, appeared with bladder pressure rises of less than 50, 75 and 100 mm Hg and of 100 mm Hg or more in 7, 27, 45 and 67% of the 45 symptomatic patients, respectively. 33% had a positive closure pressure in every UPP. 2 women without symptomatic incontinence had negative urethral closure pressures. The lowest bladder pressure rise needed for zero urethral closure pressure showed a significant negative correlation with the clinical grade of SUI and the degrees of social restriction experienced. We suggest that SUI can be classified urodynamically into minimal (lowest bladder pressure rise producing zero urethral closure pressure 100 mm Hg or more), mild (75–99 mm Hg), moderate (50–74 mm Hg) and severe forms (less than 50 mm Hg).

Cytogenetic studies were carried out on 118 couples with recurrent spontaneous abortions. Four major chromosomal abnormalities were found including two 13/14 Robertsonian translocations, one t(7;12) and one t(1;10) reciprocal translocation. The incidence of chromosomal abnormalities in this study was 3.39%, which is lower than the mean value of the published data. The clinical significance of balanced translocations in recurrent reproductive loss is discussed.

Second trimester spontaneous abortion occurring on the average 1.5 weeks after amniocentesis in 15 symptom-free women has been studied. Thus, hormone (total estriol, hCG, prolactin, and hPL) levels in amniotic fluid and maternal serum were compared to those in 60 women who also underwent second trimester amniocentesis but went to term without complications. The abortion group had significantly (p < 0.01) elevated levels of total estriol in maternal serum and in amniotic fluid, significantly (p < 0.01) increased prolactin concentration in amniotic fluid, and decreased (p < 0.05) hCG levels in maternal serum. It is suggested that these differences may relate to fetal adrenal stress response in the subsequent abortion group indicating an abnormality which may or may not be aggravated by amniocentesis.

The urinary excretion of 6β-hydroxycortisol (6β-OHF), free cortisol, and estriol was studied at 4-hourly intervals in 9 subjects between the 36th and 38th week of pregnancy. The ratio of 6β-OHF to creatinine showed a significant diurnal rhythm with a 40% fall from a peak at 06.00–10.00 h to a trough at 22.00–02.00 h. There was a significant positive correlation between the excretion of 6β-OHF and free cortisol. Estrogen excretion showed no diurnal variation. No increase was observed in the 6β-OHF:creatinine ratio in early morning urine samples from 5 subjects followed daily prior to the onset of spontaneous labour at term. Normal nonpregnant females were also found to excrete significant quantities of 6β-OHF. These findings suggest that the clinical significance of 6β-OHF levels measured in a single void sample of urine has to be reassessed in the face of such a marked diurnal variation.

Histochemical localization of pregnancy-associated plasma protein A (PAPP A) was performed on various genital and extragenital organs from fetuses, infants, and adults. Three types of PAPP A reactive cells were found: endocrine, epithelial, and hematopoietic cells. Positive endocrine organs included the decidua and the fetal adrenal and Leydig cells. Positive epithelial cells were found in actively proliferating tissues, such as fetal renal tubules, fetal intestinal mucosa, and adult proliferative endometrial glands. In bone marrow, red cell precursors were PAPP A positive. Comparison of a commercially available antiserum with one raised in our own laboratory revealed significant differences in histochemical staining. The biological relevance of these findings remains to be determined.

Scanning electron microscopy of cryofractured immature intermediate villi showed numerous intercommunicating channels in the central region of the stroma, oriented mostly parallel to the major axis of the villus. In the stromal channels, numerous Hofbauer cells were present and their aspect and position inside the channels highly suggested a motility of these cells. These morphological data strongly support the fundamental role played by the immature intermediate villus in the normal and pathological development of the human placenta.

A low molecular weight fraction of human amniotic fluid was found to be rapidly bactericidal for Escherichiacoli ATCC 33908. Incubation in an atmosphere of CO2 but not N2 diminished the antibacterial effect suggesting that loss of CO2 and change in pH may be partly responsible for the observed antibacterial activity of the low molecular weight fraction of amniotic fluid. Incubation in pH 8.5 buffers did not duplicate the rapid killing of bacteria observed in the low molecular weight fraction of amniotic fluid, suggesting that elevated pH and an antibacterial factor from amniotic fluid act synergistically. The pH change in amniotic fluid which occurs in room atmosphere may account for some of the antibacterial effect which has been reported for amniotic fluid incubated aerobically.

In order to elucidate some aspects of the efficacy of orally administered ritodrine, serum concentrations of ritodrine were determined during intravenous and oral therapy. Ritodrine serum concentrations during steady-state intravenous therapy (200 μg/min) ranged from 91.0 to 123.0 ng/ml. Serum concentrations during maintenance therapy with 120 mg ritodrine per os fluctuate between 3.2 and 30.5 ng/ml, depending on the moment of tablet intake. During oral maintenance therapy a striking correlation was found between the maternal heart rate and the serum concentration of ritodrine. The findings suggest that efficacy of orally administered ritodrine is doubtful on pharmacological grounds.
