An attempt was made to verify the hypothesis that a low-lying placenta may be a factor leading to the dynamic form of cervical incompetence. A vaginal evaluation of the cervix was made in 143 pregnant women who by means of ultrasonographic method were found to have low-lying placentas. In as many as 68 out of the 143 studied cases (47.5%), the low-lying placenta coincided with a shortening of the cervix about half its normal length or in both shortening and patency of the uterocervical canal, measuring up to 2 cm in diameter (39 cases). The statistical calculations indicate that a low-lying placenta can influence cervical behaviour and that it appears to be a risk factor of cervical incompetence.

Phenylethylamine-agarose column chromatography has been used to compare inactive renins in amniotic fluid, in control plasma during estrogen treatment and in maternal plasma during normal pregnancy. Inactive renin in all three was retained by the column and separated from renin substrate. Inactive renin in control and maternal plasma desorbed similarly and was different than that in amniotic fluid. The results suggest that during normal pregnancy the elevated concentrations of inactive renin in maternal plasma are more likely of renal than placental origin.

The delivery results of 42 infants born to 40 mothers at the gestational age of 26–28 weeks during a period of 5 years were analyzed. The study was evaluated in two periods of time: in the first period out of 15 infants born only 5 (33.3%) survived, while in the second period 21 (77.7%) out of 27 infants survived (p < 0.01). 38 infants were transferred to a neonatal intensive care unit for premature infants. Only 41 % of the infants transferred in the first period survived, as compared to 80.7% of those transferred during the second period (p < 0.01). There was no significant difference in the mean birth weight at each gestational age between the survivors and those who subsequently died in both periods of the study. In the study groups, cesarean section rate rose from 13.3% in the first period to 44% in the second. Mode of delivery, regardless of the presenting part, did not seem to influence neonatal survival. Most of the cyclic nucleotide phosphodiesterase (PDE) activity of pregnant human myometrium was found in the soluble fraction. Chromatography of this fraction on DEAE-cellulose resolved two peaks of PDE activities. Peak I hydrolyzed both cAMP and cGMP and was activated by the Ca2+-calmodulin complex. Peak II, insensitive to this complex, hydrolyzed specifically cAMP. Sucrose gradient centrifugation of the soluble fraction resolved three peaks (A, B, C) of cAMP PDE activities, and only the first two peaks (A, B) were active towards cGMP. A subsequent sucrose gradient centrifugation of peak I, previously determined by DEAE-cellulose, allowed us to restore two peaks A and B identical to those directly obtained from the soluble fraction: peak A hydrolyzes both substrates, while peak B is specific to cGMP hydrolysis. For peak II, a single large cAMP PDE activity peak is generated. Considering Ca2+ and cAMP as intracellular messengers in the control of uterine motility, the characterization of the different forms of PDE in pregnant human myometrium will be of importance in developing improved tocolytic therapy.

Blood coagulation and fibrinolysis variables have been studied in the normal puerperium in order to facilitate the decision to discontinue thrombosis prophylaxis after delivery. 16 women were followed longitudinally from the 1st to the 6th week post partum. Factor VIII activity and related antigen, fibrinogen, fibrinopeptide A, antithrombin III, plas-minogen, tissue plasminogen activator (t-PA), fast inhibitor of t-PA, α2-antiplasmin, uroki-nase inhibitors, fragment Bβ 15–42 and kallikrein inhibition were analyzed. Both blood coagulation and fibrinolysis were significantly increased during the first 2 weeks, although simultaneously increased inhibitor capacity of both systems was present. 3 weeks post partum, blood coagulation and fibrinolysis were generally normalized, although the inhibitors remained raised compared to nonpregnant controls throughout the observation period.

Aqueous extracts of human luteal tissue significantly inhibited binding of 125I-labelled human luteinizing hormone (LH) to the 2,000-g subcellular fraction of human corpora lutea. In contrast, aqueous extract of nonluteal tissue of the human ovary did not show a comparable activity to inhibit LH binding. Extracts of human corpus luteum had little or no ability to inhibit LH binding to porcine luteal homogenates. The inhibitory effect of the aqueous extract on LH binding to human luteal homogenate was demonstrated to be dose-related. The inhibitory effect of aqueous extracts of human corpora lutea on LH binding increased from the early to mid and late luteal phase of the menstrual cycle. The results of the present study suggest that there is an LH receptor binding inhibitor (LHRBI) in the 30,000-g aqueous extract of the human corpus luteum and the increase in LHRBI in the luteal tissue as the human corpus luteum ages may explain the means whereby the human corpus luteum regulates its own life span.
