We analyzed the case histories and operation protocols of 2,465 female patients who had undergone pelviscopy for various reasons at the University Clinic of Obstetrics and Gynecology, Kiel, in the years 1978–1982. 1,743 patients (71%) had reported a previous appendectomy, of whom 965 had had no other intra-abdominal operation. 657 of the latter (68.1 %) presented adhesions; these were located in the right middle abdomen in 36.4% of the cases. In comparison to a collective of 308 patients without adhesions after appendectomy, the existence of chronic lower abdominal pain was independent of the presence of adhesions (30–31 % in both groups). In 55% of the patients with adhesions who were infertile after appendectomy, periovarian and peritubal adhesions were registered. In order to avoid unnecessary appendectomy or other laparotomies with later formation of adhesions, the use of laparoscopy is recommended in all questionable cases.

The effect of methylergometrine maleate (methergin) on electrical and mechanical activities of pregnant human isthmic myometrium was examined. Methergin (10–8–10–6 g/ml) enhanced plateau potential and contraction, however, the effects were not prominent. Plateau potential and contraction were also potentiated after pre-treatment with 10–7 g/ml phentolamine (α-blocker), however 10–7 g/ml methergin further increased these effects. It is suggested that methergin might potentiate contractions by changing the pattern of action potentials and not by α-adrenoceptor.

Results of intrauterine insemination (IUI) in 39 couples are presented. Indications were male subfertility, cervical factor, unexplained infertility and immunological factor. The patients received a total of 119 treatment cycles. Nine pregnancies were achieved (23.1%) during IUI treatment. The monthly fecundity rate (MFR) during treatment was 7.6%. After stopping IUI, within a follow-up of 1 year 13 pregnancies occurred. Seven pregnancies occurred spontaneously without invasive treatment methods. The MFR was calculated to be 4.8% for this group. We conclude that couples with IUI treatment do achieve pregnancies more quickly, but IUI does not influence a patient’s eventual chance to become pregnant.

Magnetic resonance imaging (MRI) examinations were made on 9 high-risk fetuses (poor fetal growth, 3; maternal manic-depressive psychosis, 1; maternal systemic lupus erythematosus, 1; habitual intrauterine fetal death, 1; twin pregnancy with 1 fetus in distress, 1; fetal hydrocephalus, 1), ranging from 29 to 36 weeks of gestation. T1 weighting sequences were better than T2 weighting sequences for in utero fetal imaging. Fetal movement signifícantly degraded the imaging quality. In all fetuses, cerebrum, cerebellum, heart, lung, liver, stomach, bladder and extremities were identified and in 1 fetus, even the cochlea was seen. In 2, the four-chamber view of the heart was evident. The signal intensity of the lung was lower than that of the liver in T1 weighting imaging. In the case of maternal lupus erythematosus, a cavum veli interpositi was noted. In the fetus with hydrocephalus, clear images of the intracranial structure were obtained. Similarly, location of the placenta was clearly depicted. MRI is currently less than satisfactory to determine the fetal anatomy, however, it does provide unique and characteristic information that complements the superior anatomic imaging by ultrasonography.

Thirty-nine patients with hypertensive complications of pregnancy were analyzed in the third trimester with respect to erythrocyte glutathione peroxidase (GSHPx) activity. Patients with severe or superimposed preeclampsia had significantly higher activities of GSHPx than those with mild preeclampsia. We suggest that a high level of GSHPx in the severe forms of preeclampsia reflects the greatly increased peroxide load generated in this disease and is of compensatory nature, responding to the disturbed peroxide and prostaglandin metabolism.

A kinetic test (Phadebas®) was employed to determine the isoamylase activity in 77 amniotic fluid samples taken from a total of 60 high-risk pregnancies at different gestational ages. It was ascertained that in high-risk pregnancies the s-type isoamylase level frequently deviates from the common scatter range of the respective week of gestation. The measured activity of the s-type isoamylase is subsequently set in relation to the pre- and postnatal state of the newborn. The findings support the assumption that in addition to ultrasound, hormone determination and cardiotocography, the estimation of s-type amylase activity in amniotic fluid may serve as an index for fetal well-being. A reduced secretion of the parotid gland as a result of fetal stress is discussed as a possible cause of the ascertained enzymatic changes in the amniotic fluid.

Thirty-two patients with intrauterine fetal death after the 20th week of pregnancy were treated with 1–3 intramuscular injections of 500 mg of 16-phenoxy-PGE2-methylsulfonylamide (sulprostone). Intrauterine pressure records demonstrated onset of contractions within 20 min of injection and all women aborted-delivered after in mean 11 h (4–31 h). No effect on routine hematologic and plasma parameters, kallikrein-kinin and factor XII systems was observed. It is concluded that 16-phenoxy-PGE2-methylsulfonylamide is an effective and safe oxytocic agent for the induction of labor in case of intrauterine fetal death with minimal effects on the coagulation system.

A prospective, randomized clinical trial was conducted to compare the efficacy and tolerance of a single dose of 1 g ceftriaxone i.v. daily with 3 doses of 1 g cefotaxime i.v. daily for obstetric and gynecologic infections. Both agents are characterized by a wide spectrum and potent activity. Furthermore, ceftriaxone has an outstanding serum half-life of 8 h. 41 patients with pelvic inflammatory disease, pelvic or wound infections after vaginal or abdominal hysterectomy, endomyometritis and urinary-tract infection were included. Patients were monitored clinically by routine laboratory methods (erythrocyte sedimentation rate, white blood cell count and cross-reacting protein) and bacteriologically. Clinical parameters of infection were fever, local pain and/or tenderness, a sactosalpinx or pyosalpinx at palpation and cervical secretion. Clinical cure was achieved in 77.3% in the ceftriaxone and in 78.9% in the cefotaxime group, improvement in 3 (13.6 %) and 4 patients (21.0%), respectively. 2 clinical failures were seen in the ceftriaxone group. One was a severe pelvic infection following vaginal hysterectomy, which responded to the addition of metronidazole, the other was due to a chlamydial salpingitis, which was cured with a 10-day course of doxycycline. Both antibiotics were well tolerated. Our results suggest that for obstetric and gynecologic infections a single 1-gram dose of ceftriaxone is equally effective as three 1-gram doses of cefotaxime.

Distinct expression patterns of keratins reflect different epithelial prototypes and different epithelial differentiation pathways. In order to get evidence of the expression of keratins in a hormone-dependent tissue in man, we investigated 70 epithelial tissue specimens by means of SDS-gel electrophoresis and immunoblot. Every tissue probe belonged to different generation phases such as sexual maturity, climacteric and postmenopausal period and pregnancy. Keratins l(avb), 4(avb), 5(avb), l0(avb), 13, 14 were detected regularly whereas keratins 6, 16, and 19 are expressed irregularly. During all generation phases, with the exception of pregnancy, the keratins 1 and 10, associated with the epidermal differentiation, and keratins 4 and 13, associated with the differentiation to a non-keratinizing stratified epithelium, are synthesized simultaneously. However in pregnancy, i.e. pregnancy at term of delivery, only K4/K13 are expressed, while K1/K10 are below the detection limit. In terms of keratin expression vaginal epithelium represents a mosaic of differentiation. Only in pregnancy the ‘pure’ type of a nonkeratinizing epithelium is seen, apparently as a result of the high concentration of estrogens.

Chlamydia trachomatis infection is the most common of the sexually transmitted diseases. In this study of screening for C. trachomatis infection in the Turkish female population, Chlamydia IgG enzyme immunoassay (Ch IgG EIA) was used to detect the exposure to C. trachomatis. Ch IgG EIA was studied in 469 women attending the Gynecology and Obstetrics Clinic of the University of Gazi School of Medicine. Ch IgG EIA was found positive in 35 (7.46%) blood specimens. The highest ratio of seropositivity was detected among the infertile population (11.17%).
