Characteristics of intrauterine activity in response to nipple stimulation were studied in 9 healthy pregnant subjects at term. All patients showed an increase in uterine activity varying from 10 to 730%. In 1 subject, marked uterine hypertonous was noted which resulted in profound fetal bradycardia. This response was transient, lasting 5.5 min, and fetal outcome unaffected. We believe that nipple stimulation is associated with a higher incidence of increased uterine activity in terms of frequency, intensity and basal tone, than has been previously recognized. This uterine hypertonus may affect the fetal heart rate adversely indicating the need to exercise caution when adopting this manoeuvre in late pregnancy, particularly when there is already evidence of fetal compromise.

Zinc has long been known to be an essential element for the growth and development of living things. In fact, zinc is present or participates in a wide variety of metabolic processes including the metabolism of carbohydrates, lipids, proteins and polynucleotides. The purpose of our study was to determine some aspects of the involvement of zinc in intrauterine growth and development of the rat. Thirty female Wistar rats were assigned at random to 1 of 3 dietary treatments. Group A received a normal diet, group B received a zinc-deficient diet and group C received a zinc-deficient diet plus a supplement of zinc in the drinking water. Daily food consumption, weights and plasmatic zinc levels were measured on days 1,10 and 20. Fetal weights on day 20 were also recorded. The results showed a significant difference in daily food consumption and weight gains of the dams between the group receiving the normal diet (group A) and the other two groups. Fetal weights showed a different pattern, there was a significant difference between the unsupplemented zinc-deficient group B and the other two groups which led us to think that the daily food consumption of groups B and C, even though enough to produce lower weight in the dams, was not sufficiently low to produce fetal malnutrition and that it was the adequate consumption of zinc which allowed the fetuses of group C to have similar weights to those in group A, emphasizing the requirement of this metal for normal growth.

Ultrasonography was performed to time ovulation in 30 patients participating in program for artificial insemination with donor semen (AID). During this study (10 months) 61 scans were carried out where one or more follicles were visible. The mean number of treatment cycles per patient was 2.8. Insemination with frozen donor semen was performed if a clear and mature follicle was detectable. The mean follicular diameter in successful inseminations was 20.1 ± 0.8 mm, ranging from 16 to 23 mm. Twelve patients became pregnant, 9 (75%) of them after three treatment cycles. The mean time to conception was 2.75 cycles and the most successful day of the menstrual cycle was day 13. Ovulation was induced in 8 (56.7%) patients who conceived. Ultrasound is a noninvasive, quick and reliable method in the detection and timing of ovulation in most AID patients.

Cervical mucin was quantitated in individual samples collected daily during midcycle in 14 normally menstruating women. Quantitation was performed with a recently developed lectin-mediated light scattering (nephelometry) method after separation of soluble proteins from the mucin. Furthermore, mucin was quantitated in individual samples in another group of 12 women from whom cervical secretion samples had been collected hourly from 8.00 to 12.00 a.m. There were significant differences between cycle days (p < 0.05; two-way analysis of variance) for total amounts of mucin secreted and for mucin concentrations. But during repeated sample collection, only the total amounts of mucin secreted and not the concentrations were affected. From trend analysis data it could be concluded that both mucin contents and water contents increased at midcycle.

Endocrine studies were carried out on 3 patients with streak gonad syndrome, 2 of whom had slight proliferative vaginal smear patterns, and the 3rd had slight hirsutism with increased muscle mass but an atrophic type vaginal smear pattern. Simultaneous sampling of peripheral, adrenal and streak gonadal venous blood revealed mildly increased testosterone secretion by the streaks in the 2 patients who had proliferative cytohormonal patterns. Histologic examination of their streak gonads showed hilus cell hyperplasia. It might be suggested that hilus cell hyperplasia was the source of the increased testosterone secretion in these 2 patients and thus resulted in slight maturation of the vaginal epithelium.

The concentrations of plasma progesterone and androstenedione were investigated in 13 postmenopausal women with malignant ovarian tumors who showed no signs of other malignant or endocrine disease. FIGO stages III and IV were represented and histological types were mainly Ic and V. Tumor volumes were generally evaluated once a month using bimanual rectovaginal palpation under anesthesia. Blood samples were drawn for progesterone and androstenedione radioimmunoassay at every admittance to hospital. In 12 of the 13 patients the plasma levels of progesterone and in 10 the plasma levels of androstenedione decreased in parallel with the reduction in tumor volume and increased again when recurrence occurred. The plasma levels at diagnosis of recurrence of tumor and the levels when the largest tumor volume was demonstrated were compared to the preceding values and found to be significantly higher. In order to evaluate if progesterone or androstenedione could predict recurrence of tumor, plasma levels at certain intervals before recurrence were compared to the preceding value. Significant rise was considered to have occurred when plasma concentration had risen to twice the coefficient of variation of the assay above the previous value and contined to rise. Plasma progesterone indicated recurrence of tumor in 8 cases at least 1 month before recurrence was palpable. Plasma concentration of androstenedione was found to be useful in predicting recurrence in 5 cases 1 month or more before recurrence was palpable. A group of postmenopausal women with disseminated malignant disease of nongenital type was compared to the control groups. Androstenedione and albumin concentrations were significantly lower than in the control groups. The progesterone concentration was, however, not differed from the controls.

A 37-year-old G1-P1 was diagnosed by ultrasonography at 26 weeks of gestation as having an abnormally large placenta with hemangiomas and a fetus associated with exomphalos. Placental protein 5 levels were relatively high in placental protein levels in maternal serum. The infant, delivered by cesarean section at 34 weeks, had the typical clinical features associated with Beckwith-Wiedemann syndrome. The abnormally large placenta weighed 1,492 g, measured 25 × 25 × 5.1 cm, and featured multiple hemangiomas. Microscopic placental features included edematous villi, increased fibrin deposition, intervillous thrombi, and multiple angiomatous and cellular chorangiomas.
