摘要:
Viral gastroenteritis is an important global problem that is being actively investigated. This year, stimulating new concepts regarding pathogenesis and immunity of the major viral pathogen of the gastrointestinal tract, rotavirus, were reported. In addition, highly promising results from phase 3 viral vaccine trials suggest that an effective vaccine for rotavirus will be available in the next several years. The range and diversity of human caliciviruses continues to be defined as more isolates are characterized. The ability to produce recombinant virus-like particles of human caliciviruses has enhanced diagnostic capability and enabled more extensive epidemiologic studies. The roles that human astrovirus, adenovirus, and newer agents, such as Aichi virus, play in diarrheal disease continue to be investigated.

摘要:
The 1993 cryptosporidiosis outbreak in Milwaukee was the largest waterborne outbreak ever recorded in the United States. It generated tremendous nationwide concern for the potential threat of parasites, primarilyCrytptosporidiumspp andGiardia lamblia, to drinking water. Since then, there has been increased interest in the development of techniques to detect these parasites in drinking water. Several investigators are seeking to characterize species and strain differences and to correlate the differences with virulence and varied manifestations of clinical disease in humans. Attempts to delineate the relative importance of different risk factors for illness caused by these two parasites and to develop effective measures for the protection of the water supply are also ongoing. Improved and inexpensive diagnostic methods are still needed for all protozoan and helminthic parasites. The past year has seen the application of commercially available antigen detection kits and molecular biologic techniques to enhance diagnosis of a number of parasites. Laboratory and clinical attempts at identifying effective therapy for cryptosporidial infection have not resulted in any major breakthroughs.

Helicobacter pylori infection continues to generate considerable interest in the medical and scientific community. There is now overwhelming evidence to confirm thatH. pyloriplays an etiologic role in the development of chronic active gastritis, peptic ulcer disease, and gastric cancer. Tremendous progress continues to be made in identifying the pathogenic mechanisms by which chronicH. pyloriinfection promotes the development of gastroduodenal disease. This review outlines the recent contribution to the literature and focuses on the epidemiology, diagnosis, and therapy ofH. pylori, an update on vaccine development, and new insights into the pathogenic mechanisms resulting in the development of gastroduodenal disease.

摘要:
This review highlights recent reports on new developments in the treatment of gastrointestinal infections. The strong association ofHelicobacter pyloriwith recurrent ulcer disease stimulated several investigators to reassess the traditional approach to evaluation and treatment of the large number of patients with symptoms of dyspepsia. Reports on treatment of bacterial intestinal and intra-abdominal infections emphasize the continuing major problem of antibiotic resistance. Studies using percutaneous drainage in the treatment of intra-abdominal abscesses further establish the role of this minimally invasive technique. Finally, we highlight a report describing success with intralesional interferon as a treatment for anal warts.
