Bisher wurde davon ausgegangen, daβ Leukozyten in gefrorenem Frischplasma (FFP) den Einfrier-/Auftauvorgang nicht überstehen. In einigen Fallen wurden jedoch Graft-versus-Host-Reaktionen im Zusammenhang mit Plasmatransfusionen beobachtet. Weiterhin finden sich nach dem Auftauen noch vitale Zellen mit zum Teil proliferativer Fähigkeit im Stammzellassay. In einer prospektiven Studie wurden 40 mittels «Top-and-Bottom»-Präparation hergestellte Frischplasmen auf ihren Leukozyten- und Thrombozytengehalt vor [Leukozyten: 4.0 × 106 (1-10)/Plasma; Thrombozyten 1.6 × 109 (0.1-9)/Plasma] und nach [Leukozyten: 2 ×105 (0.2-24)/Plas-ma;Thrombozyten: 3 ×108 (0.6-11)/Plasma] einer Gefrierphase (-40 °C für minde-stens 2 Wochen) und nach einer anschlieβenden Leukozytenfiltration untersucht. Die Thrombozyten wurden in der Neubauer-Kammer (Verdünnung 1:10) und die Leukozyten in der Nageotte-Kammer (Verdünnung 1 : 5 in Ammoniumoxalat) gezählt. Die Faktoren V und VIII wurden durch die Filtration nicht beeinfluβt. Der Gehalt an Plättchenfaktor-4 (85%) und β-Thromboglobulin (35%) wurde durch die Filtration signifikant reduziert. Auβerdem wurden zellreiche Frischplasmen (n = ll) hergestellt und ohne vorheriges Einfrieren gefiltert, um die Depletionskapazität des Filters zu verifizieren. Die Ausgangskonzentrationen an Leukozyten konnten dabei um mehr als 4 Log10)-Stufen gesenkt werden [von (6,0 ± 2,6) ×108 Leukozyten/ Plasma auf (2,6 ± 0,4) × 104]. Die Filtration von Plasmen bietet die Möglichkeit, Präparate mit einem geringen Restzellgehalt herzustellen; ob das Risiko von Allo-immunisierungen und die Infektiosität zu senken sind, bleibt noch zu untersuchen. Inhibition of phosphatases 1/2A (ppl/2A) by okadaic acid (OA) prevents thrombin-or collagen-stimulated platelet aggregation, serotonin and ATP release, as well as intracellular Ca2+ increase. OA is added to 12 platelet concentrates (PCs) prepared from 500 ml whole blood. Microcystin LR (MC), a further ppl/2A inhibitor that does not penetrate the cell membrane, is added to 9 PCs. 9 PCs remain untreated. Storage of PCs is carried out on a horizontal agitator for 7 days. Samples are taken daily to estimate WBC, RBC, PLT, cAMP, cGMP and serotonin. 12 h after OA addition, a dose-dependent drop in platelet counts is seen, accompanied by a corresponding increase of particles with sizes of WBC and RBC. Increased WBC and RBC counts are artefacts brought about by platelet aggregation. From day 4 of storage, serotonin is released massively, cAMP consumption is reduced or stopped, and cGMP concentration increases slightly. In untreated and MC-treated PCs no similar reactions are observed. It seems that the inhibition of extracellular ppl/2A (ectophosphatases) prevents platelet activation. The inhibition of intracellular ppl/2A (endophosphata-ses) presumably enhances platelet activation. The different results between OA- and MC-treated PCs lead to these conclusions because, depending on dose and time, OA penetrates platelet membranes, MC does not. Therefore the results of the study give rise to the suspicion that ppl/2A control two sites of signal transduction in platelets of stored PCs: extracellular ppl/2A are included in platelet activation, while intracellular ppl/2A mediate inhibition of activated platelets. Inhibition of ectophosphatases and activation of endophosphatases probably facilitate PC storage.

Hintergrund: Das Gelzentrifugationssystem (ID-Microtyping-System®, Fa. Diamed, Bensheim, BRD) ist eine von mehreren neuen, in den letzten Jahren kommerziell verfügbar gewordenen Methoden für den Nachweis von Antikörpern gegen erythrozytäre Antigene. Material und Methodik: Während eines Jahres führten wir in unserem transfusions-medizinischen Routinelabor den Antikörpersuchtest in der Gelzentrifugationstechnik mit 3 Ansätzen durch: 1. NaCl-Karte (Raumtemperatur) mit unbehandelten Erythrozyten; 2. NaCl-Karte (37 °C) mit papainvorbehandelten Erythrozyten und 3. Liss-Coombs-Karte (37 ¤C) mit unbehandelten Erythrozyten. Ergebnisse: Beim zweistufigen Papaintest in der Gelkarte konnten wir mehrere interessante Beobachtungen machen: 1. Die Sensitivität des Papaintestes übertrifft in einigen Fallen die aller anderen von uns untersuchten Testverfahren, und zwar in der Gel- und Röhrchenzentrifugationstechnik. 2. Es finden sich Reaktionen, deren Spezifität patienteneigenen Antigenen (des Rhesus- oder Kidd-Systems) entspricht, ohne daß der direkte Coombs-Test positiv ware und ohne daß klinische Hinweise auf eine Hämolyse vorliegen. 3. Manche Seren zeigen Reaktionsmuster, die eine abgrenzbare Spezifität zu besitzen scheinen, ohne daß sie einem der beim Panel des Herstellers angegebenen Antigene zuzuordnen wären. Schlußfolgerungen: Vor einer allgemeinen Empfehlung für die Antikörpersuche mit dem zweistufigen Papaintest im Gelzentrifugationssystem sind weitere Untersuchungen zur klinischen Wertigkeit der ausschließlich mit dieser Methode nachweisbaren Antikörper erforderlich. Experiments were carried out with dog erythrocytes as an animal model for human erythrocytes. Caused by hemolysis 30% of the erythrocytes formerly cryopreserved with hydroxyethyl starch (HES) (200000/0.5; 12,5%) could not be labeled with 51Cr. All the cryopreserved labeled erythrocytes showed the same in vivo survival as labeled fresh red cells. After autologous substitution of 10% of the dog’s blood by the HES cryopreserve, the HES plasma level decreased, following a first-order kinetics with t½ = 4 h. Concerning the blood flow behavior, a transient reduction by the cryopreserved erythrocytes was seen, while the HES had adverse as well as beneficial effects. We present a new method for deglycerolizing cryopreserved autologous blood. With the Cell-Saver (Hemonetics) the thawed blood units are processed directly in the operation theatre by the anesthetist. The quality of the so prepared blood is examined. As to the removal of glycerol, free hemoglobin, leukocytes, and potassium a quality is achieved that compares to the standard procedure. Further we investigated the posttransfusion period of our patients. There was no impairment in renal function and coagulation. No difference was seen in the need of allogeneic blood transfusion compared to the patients that received liquid stored autologous blood. We conclude that processing and washing of cryopreserved autologous blood with the Cell-Saver is feasible and safe.

Hintergrund: Intravenös verabreichbare Immunglobuhnpräparate enthalten nicht nur die in der Regel deklarierten Antikörper gegen pathogene Mikroorganismen, sondern auch andere Antikörper der Blutspender, z. B. gegen erythrozytäre Antigene. Material und Methodik: In unserer Untersuchung wurden 14 Präparate von 7 Herstellern (insgesamt 40 Chargen) auf das Vorhandensein von Isoantikörpern und irregulären Blutgruppenantikörpern untersucht. Um die Ablesbarkeit zu verbessern, setzten wir die Gel-Zentrifugations-Technik (ID-Microtyping-System®, Fa. Diamed, Bensheim, Deutschland) ein. Ergebnisse: Die höchsten Isoantikörpertiter betrugen (bei 8 Chargen) 1:32 oder 1:64 in der Liss-Coombs-Phase. Irreguläre Antikörper wurden in 5 Präparaten (höchster Titer 1:8) gefunden. Schlußfolgerungen: Die in den Präparaten enthaltenen Isoantikörper können blutgruppenserologische Untersuchungen beeinflussen. Am Beispiel eines Neugeborenen weisen wir auf die potentielle Gefahr von Fehlinterpretationen hin, wenn nach Immunglobulingabe ein positiver direkter Coombs-Test gefunden wird. Wir halten es daher für sinnvoll, bei Kindern und Erwachsenen 1. den direkten Coombs-Test vor einer Immunglobulintherapie durchzuführen und 2. das Eluat nicht nur mit einem Panel von 0-Erythrozyten, sondern gegebenenfalls auch mit A- bzw. The basic mechanisms of the freezing of cell suspensions are discussed in terms of solute enrichment and the resulting osmotical consequences. The shrinkage kinetics of the cell volume and the influence of the cooling rate play a crucial role regarding cell damage due to solution effects or intracellular ice formation. The use of cryoad-ditives permits a substantial reduction of the optimal cooling rate, due to different mechanisms for non-penetrating low molecular weight substances and polymers such as hydroxyethyl starch.

Objective: Review of different aspects of the primary interaction of complement with blood platelets in immunological reactions and the effect on platelet activation in healthy people and patients. Data Sources and Selection Criteria: Relevant original papers and review articles mainly of the English-written literature. Results: Besides their major role in hemostasis and wound healing, blood platelets are involved in immunological reactions. They are not only able to interact with IgG through Fc receptors (FcR), they also react with complement components. This review summarizes interactions of complement with mainly human platelets. Such interactions may occur through complement receptors of the plasma membrane (e. g. Clq receptor, complement receptors 2 and 4), but also in a receptor-independent way including activation of the platelet by the membrane attack complex of complement C5b-9. In addition, activation of complement at the surface of the platelets may be induced after binding of anti-platelet antibodies to membrane glycoproteins (e. g. GpIIb/IIIa, GpIb/IX) or after binding of platelet-nonspecific immune complexes via FcR. Complement activation in turn may be regulated by various means including specific plasma or membrane proteins [e. g. decay-accelerating factor (DAF), membrane cofactor protein (MCP), membrane inhibitor of reactive lysis (MIRL), C8-binding protein (C8bp, homologous restriction factor hrf)]. As a further way of self-protection against complement attack, platelets may actively release C5b-9, deposited at the surface as C5b-9-enriched membrane vesicles. Conclusions: Two lines of interaction of platelet with complement can be distinguished. On the one hand, platelets are equipped with membrane proteins which protect them from complement attack against themselves. On the other hand, membrane receptors for activated complement components as well as for IgG are expressed on the surface, which enable the platelet to intervene in immunological reactions. This property varies between platelets of different species and needs further investigation also in view of the platelet as an intersection between immunology and hemostasis.

Aufgrund epidemiologischer und klinischer Befunde mehren sich Hinweise auf eine Bedeutung hämorheologischer Mechanismen in der Pathogenese der Atherosklerose. Meβgröβen der Blutrheologie wie Plasmaviskosität und Erythrozytenaggregation werden von der Konzentration hochmolekularer Bestandteile im Plasma beeinfluβt. Extrakorporale LDL-Eliminationsverfahren wie spezifische Immunadsorption (SIA) oder Kaskadefiltration (CF), die zur Prävention oder Therapie der vorzeitigen Atherosklerose bei Patienten mit schwerer familiärer Hypercholesterinämie (FH) eingesetzt werden, manipulieren entsprechend ihrer Eliminationscharakteristik die Plasmazusammensetzung über einen weiten Bereich. Wir untersuchten die Akuteffekte von SIA und CF auf hämorheologische und laborchemische Parameter bei 8 Patienten mit heterozygoter FH unter chronischer LDL-Aphe-resetherapie. SIA zeigte eine spezifische effektive Elimination von LDL-Choleste-rin sowie unspezifische Verluste der übrigen Plasmabestandteile. Bei CF kam es zu einer selektiven Absenkung nicht nur von LDL, sondern auch von Fibrinogen und weiteren hochmolekularen Plasmaproteinen. Alle untersuchten hämorheologischen Parameter wurden von beiden Verfahren deutlich und signifikant abgesenkt. Im Vergleich zu SIA senkte CF die standardisierte scheinbare Vollblutviskosität (Hkt 45%) sowie die Plasmaviskosität und insbesondere die Erythrozytenaggregation starker ab.
