摘要:
In this review the authors cover compression neuropathies localized to the wrist and hand. Specifically, these include carpal tunnel syndrome, ulnar tunnel syndrome, Wartenberg's syndrome, distal posterior interosseous nerve syndrome, and bowler's thumb. For each of these neuropathies, the relevant anatomy, presentation, and treatment are discussed. Correlations to specific sport-related injuries are also discussed with relevant options of management.

摘要:
Tendon injuries in athletes' hands can be disabling and may lead to a chronic disability. These injuries may occur in the professional or recreational athlete and involve the flexor and extensor tendons of the hand and wrist. Overuse syndromes may be another cause of tendon injuries to the hand and wrist. Correct diagnosis of all these entities requires knowledge of anatomy, pathophysiology, and commonly occurring injuries to the athlete's hand and wrist. Treatment may be non-operative or surgical and tailored to the specific injury sustained and the athlete involved to yield the most reliable functional result.

摘要:
Complete tears of the ulnar collateral ligament (UCL) of the thumb are often complicated by the Stener lesion, the interposition of the adductor aponeurosis between the ruptured ends of the UCL. The Stener lesion prevents healing of the UCL and, if left untreated, results in continued pain and weakness with pinch. Arthroscopic reduction of the Stener lesion has been described and is a minimally invasive alternative to open repair of UCL injuries in athletes.

Acute and chronic wrist problems can significantly limit an athlete's performance. Relatively minor injuries may significantly affect performance. Wrist arthroscopy is an integral part of the assessment and treatment of many wrist disorders; it is minimally invasive, has low morbidity, and, more important, is diag-nostically precise. Although patient selection and the exact timing of operative intervention may be controversial, wrist arthroscopy minimizes time away from sport and potentially lengthens the time an athlete can participate in sport.

An injury to the triangular fibrocartilage complex (TFCC) is one of the most common causes of ulnar-sided wrist pain in the athlete. The TFCC functions to stabilize the ulnar side of the wrist, as well as the distal radioulnar joint, and contributes to the load-bearing capacity of the upper extremity. Acute injuries are initially treated with immobilization, rest, and symptomatic relief. The emergence of advanced techniques in wrist arthroscopy has enhanced the surgical armamentarium. Central TFCC tears are débrided, whereas peripheral avulsions, both radial and ulnar, can be repaired arthroscopically. Chronic TFCC injuries are seen in older athletes and represent a continuum of injury and degeneration. Treatment involves débridement, possible distal ulnar resection, and possible lunotriqetral stabilization. Rehabilitation protocols focus on an adequate period of immobilization to allow soft tissue healing, followed by controlled, progressive range of motion and strengthening exercises.

Since midcarpal instability was originally described in 1981, much has been learned about this clinical entity. Although relatively uncommon, mid-carpal instability is becoming increasingly recognized as our level of knowledge and awareness increases. The pathophysiology is thought to be caused by congenital laxity, gradual attenuation, or traumatic disruption of key carpal ligaments, which leads to a hypermobility of the intercalated proximal row segment and a “catch-up clunk” as the patient brings his or her wrist into ulnar deviation. Patients primarily complain of painful clunking with ulnar deviation activities. The midcarpal shift test is the premier clinical diagnostic test for identifying a patient with midcarpal instability, and reproduction of the painful clunk with this test is considered positive for the disorder. Most patients with midcarpal instability respond to conservative measures that include anti-inflammatories, avoidance of aggravating activities, special splints, and physical therapy. If these measures fail, limited intercarpal arthrodesis or dorsal capsulodesis are described surgical techniques for eliminating the painful clunk in this disorder.

Wrist arthroscopy is increasingly becoming recognized as an important adjunct in the management of displaced intra-articular distal radius fractures. Anatomic restoration of the articular surface is possible under bright, magnified conditions with minimal soft tissue disruption. Osteochondral loose bodies and associated intracarpal soft tissue lesions that may not be apparent radiographically are detected and simultaneously managed. A method of management for displaced distal radius fractures using wrist arthroscopy and a combination of percutaneous and limited open reduction techniques is described.

Fractures of the distal radius are common and account for approximately 75% of all fractures of the forearm. Young athletes involved in high-velocity injuries to the wrist often sustain comminuted intra-articular distal radius fractures, which can produce great challenges to the physicians treating them. The comminution of the cortex and distal fragment frequently involve the distal radioulnar joint and radio-carpal joint, and inadequate treatment may result in functional limitation and chronic pain. The authors describe a systematic approach to evaluation and treatment of distal radius fractures, the indications and techniques for various types of surgery, and potential complications.

Patients who suffer intercarpal ligament injuries of the wrist can be treated by other soft tissue reconstruction or limited or total arthrodesis depending on the particular problem. A description of each of the partial and total wrist arthrodesis indications for athletes is given, including overall outcome parameters. Technical components of each of the types of surgery are also provided.

Fingertip injuries are frequent occurrences in athletes. Direct and local causes account for most of the lesions. Neurologic, vascular, and tendinous structures may also be involved more proximally, with terminal symptoms presenting at the fingertip. Thus, it is important to maintain awareness of the functional and physiologic nature of athletics to encompass the entire upper extremity, even if the injury is localized to the fingertip.
