摘要:
The hip is a unique joint that possesses both mobility and stability because of its anatomic configuration. This joint is crucial to athletic activities involving the lower extremity, such as running, jumping, and kicking, as well as to the generation and transference of forces for activities involving the upper extremity. The tremendous loads that the hip withstands result from muscular, gravitational, and joint reaction forces inherent in weightbearing. This article reviews the anatomy and biomechanics of the hip as a foundation for the evaluation and treatment of athletes with hip disorders.

Hip pain in the athlete may present with confusing clinical symptoms that do not reflect a specific soft tissue or osseous injury. While plain radiographs are recommended as the initial diagnostic imaging test, they are relatively insensitive to many of the more common injuries, including stress fracture. Bone scintigraphy may be helpful in disclosing multiple sites of osseous injury, but it is limited by poor soft tissue contrast and specificity. Magnetic resonance (MR) imaging has supplanted traditional imaging tests as the most comprehensive assessment of the cause of hip pain in the athlete, including stress fractures or reaction, muscle injuries, and subtle labral and cartilage injuries. In addition to traditional orthopedic causes for hip pain, certain nonorthopedic causes should be considered, including hernia formation and nerve entrapment syndromes. Musculoskeletal ultrasound, given its ability to actively load muscle–tendon units, may be efficacious in detecting dynamic tendinopathy and occult hernia formation.

Dislocation of the hip is a rare injury in sport. The deep bony anatomy and strong ligamentous and muscular support provide significant protection for the hip joint and contribute to the low incidence of this injury. However, with the significant size, speed, and strength of some athletes and the prevalence of violent bodily contact in sport, severe injuries, such as hip dislocations, do occur. The high forces required to dislocate a hip place the hip joint at high risk for severe associated injuries and complications. Accurate diagnosis, proper evaluation, and timely intervention are crucial to manage a hip dislocation and its associated injuries, avoid complications, and obtain the best possible outcome.

Femoroacetabular impingement is a cause of groin pain, which frequently afflicts young, active patients. Patients with irregularities in the morphology of the proximal femoral head–neck junction are at risk for developing impingement of the acetabular labrum. Furthermore, the cam effect causes compression and avulsion of acetabular cartilage and can result in irreversible degenerative changes in the joint of these relatively young patients. Understanding the anatomy responsible anterior impingement and disorders of the acetabular rim is essential for joint preservation. Arthroscopic debridement may alleviate acute labral symptoms but does not address the underlying osseous anatomy; treatment should include osteoplasty of the proximal femur to improve the anterior head–neck offset and allow clearance of the anterior acetabular rim. A case is presented to highlight the presentation, exam, diagnosis and treatment recommendations for patients with anterior femoroacetabular impingement.

Tears of the acetabular labrum are becoming recognized as a source of hip pain in the athlete. The diagnosis of acetabular labral tears is often delayed, leading to protracted periods of hip pain. In the athlete, symptomatic tears may lead to prolonged periods of missed play. Long-term, these lesions may results in degenerative chondral lesions and ultimately to degenerative joint disease. Treatment for labral tears entails initiation of early nonoperative therapy, with arthroscopic labral debridement reserved for refractory cases. This paper reviews the current knowledge on this subject, including the anatomy, function, and mechanism of injury of the labrum, and focuses on the history, diagnosis, and treatment guidelines for labral tears in the athlete.

The indications for hip arthroscopy fall into two broad categories. In one, arthroscopy offers an alternative to traditional open techniques previously used for recognized forms of hip pathology, such as loose bodies or impinging osteophytes. In the other, arthroscopy offers a method of treatment for disorders that previously went unrecognized, including labral tears, chondral injuries, and disruption of the ligamentum teres. Most athletic injuries fall into the latter category. In the past, athletes were simply resigned to living within the constraints of their symptoms, often ending their competitive careers, diagnosed as a nebulous groin injury. Based on the results of arthroscopy among athletes, it is likely that many of these careers could have been rejuvenated with arthroscopic intervention.

Athletic hip injuries represent a wide spectrum of stress placed either acutely or chronically on the bony and soft tissue structures of the lower extremity. Hip injuries tend to occur less commonly than other lower extremity injuries, but because of the relatively deep structures involved, diagnosis and treatment can be a challenge. It is important to recognize two broad categories of hip injury in the athlete: avulsion injuries and stress fractures. Avulsion injuries tend to occur in the adolescent athlete and involve an acute injury or trauma. Stress fractures can occur in the older athlete as well and tend to be of a more chronic nature. Often, there will be a period of discomfort with activity followed by a sudden worsening of symptoms, indicating progression of a stress injury. This article focuses on the more common injuries of the athletic hip and discusses strategies of diagnosis and treatment.

The hip joint is an intricate and complex mix of bone and soft tissues, which can make the diagnosis of hip injuries difficult and confusing. Athletic hip injuries are less common than injuries to other parts of the lower extremity. Hip problems, however, represent a significant risk for time-loss from sport secondary to the need for extensive rehabilitation before the athlete can return to full function. The most common soft tissue injuries of the hip joint are muscle strains and contusions. More specific and unique diagnoses include piriformis syndrome, snapping hip syndrome, bursitis, gluteus medius syndrome, and hip pointers. Identifying these problems with focused physical examinations can speed an athlete's safe return to sport. Most soft tissue problems about the hip can be treated conservatively if the specific diagnosis is targeted with focused rehabilitation techniques. In rare cases, surgery may be necessary to treat the problem and return the athlete to sport.
