In the following article, the authors review the current state of our knowledge of sleep abnormalities associated with mental and addictive disorders. They first discuss scientific advances in understanding sleep in chronobiology, sleep physiology, and genetics. The bulk of the article describes specific types of sleep disruptions associated with psychoses, mood disorders, anxiety disorders, eating disorders, somatoform disorders, adjustment disorders, dissociative disorders, and addictive disorders. The article concludes with a discussion of sleep issues related to life cycle and gender. The extensive and well-documented information on the role of sleep disorders in mental and addictive disorders should help clinicians diagnose, treat, and even prevent comorbid sleep and mental disorders.

The authors review the latest information on the evaluation and treatment of insomnia disorders. They describe the organization of sleep disorders in DSM-IV and present an algorithm for distinguishing patients with insomnia from those with other types of sleep-related symptoms. The questions to be considered in the initial evaluation of an insomnia complaint and the role of sleep diaries and polysomnography are reviewed. The authors explain the often confusing differential diagnosis of sleep disorders (e.g., when the insomnia complaints associated with a mood disorder would warrant an additional sleep disorder diagnosis). They then review the treatment of insomnia, including behavioral and psychotherapy treatments (sleep hygiene instruction, sleep restriction therapy, stimulus control therapy, cognitive therapy, and relaxation training), pharmacological treatments (including benzodiazepines and antidepressants), and other somatic treatments (e.g., bright light). The article ends with a discussion of which treatments are most appropriate for primary insomnia, insomnia related to another mental disorder, insomnia due to a general medical condition, and substance-induced insomnia.

The authors review the latest information on the diagnosis and treatment of social phobia. They discuss how to recognize generalised and performance subtypes of social phobia, how to differentiate normal shyness from social phobia, and describe the most common comorbid disorders. They then review the cognitive -behavioral treatment of social phobia and give a detailed description of cognitive-behavioral group treatment (CBGT). The article concludes with a detailed discussion of pharma-cotherapy for social phobia, including how to select a medication, appropriate dosing patterns, how to manage side effects that may be particularly troublesome for patients with soical phobia, and how and when to combine cognitive-behavioural therapy and medication.

The authors present a suggested standardized documentation record that they hope will eventually achieve wider usage, especially in private practice settings. They first discuss current problems caused by a lack of standard documentation and the influence of managed care and risk management on record-keeping requirements. The advantages that can result from using a standardized documentation system are then reviewed, including the delivery of better organized and integrated care, improved presentations to case managers, enhanced communication among clinicians, and reduced malpractice exposure. The authors then present one component of their proposed modular documentation system, the Initial Evaluation Form, and give a detailed annotation concerning how it should be used. Readers are requested to try the form and provide feedback to improve it. Additional components of this proposed documentation system will be presented in upcoming articles.
