The efficacy of low doses (10 to 20 mg daily) of doxepin in the treatment of depressive disorders in elderly inpatients was assessed by a double‐blind study in 24 patients. The patients were treated for a three‐week period to test for an early response. The Hamilton Depression Scale and the Geriatric Depression Scale were used to quantify symptoms of depression. The patients treated with doxepin had a significantly greater reduction in depressive symptoms than did those who received a placebo. No side effects were found and there were no major differences in the degree of physical dependency between the doxepin and placebo groups. A depressive disorder is a common occurrence among elderly inpatients and the effectiveness of low dose doxepin therapy without demonstrable side effects argues for the active treatment for this condition. Plasma somatomedin C (SmC) concentration was compared in three groups of men: 58 healthy independent men aged 26 to 45 years; 28 independent men aged 52 to 87 years; and 50 male nursing home residents aged 51 to 95 years. Somatomedin C (mean ± SE) was 0.73 ± .04 U/mL, 0.41 ± .03 U/mL, and 0.33 ± .03 U/mL in these three groups, respectively (P<.05 for the differences between all three groups). Somatomedin C less than 0.25 U/mL, a range consistent with severe growth hormone deficiency in children, occurred in 0% of the independent younger men, in 15% of the independent elderly, and in 37% of the nursing home men. Somatomedin C was inversely correlated with age in the independent elderly, but the excess of hyposomatomedinemia in the nursing home men was not explained by age. J Am Geriatr Soc 34:427–430

The present study was performed to obtain basic information as to the influences of two fundamental variables, age and blood pressure, on the circulatory responses during the Valsalva maneuver. Although a positive linear relationship between age and resting systolic blood pressure was present in the study population of 90 normal and uncomplicated hypertensive subjects, the pressure overshoot and the change in R‐R intervals during phase 4 in the Valsalva maneuver were significantly inversely related to age or resting systolic blood pressure independently of each other variable. Furthermore, age was related negatively to the maximum R‐R interval in phase 4 and positively to the decline of blood pressure post‐Valsalva release in phase 3 even after adjusting for the effect of blood pressure, but age‐independent relationships between these two indexes and resting systolic blood pressure were not statistically significant. The data indicate that changes in circulatory responses to the Valsalva maneuver occur with aging and high blood pressure, suggesting that the sympathetic vasoregulation and the baroreflex control of heart rate in the maneuver are impaired by these two variables. Such alterations appear to be more strongly related to age than to blood pressure level. J Am Geriatr Soc 34:431

As life expectancy increases, so do the problems of the elderly population. One major problem is that of protein calorie malnutrition; another is the susceptibility of this population to pressure sores. The purpose of this study was to determine if a correlation exists between deteriorating nutritional status and the development of pressure sores. The nutritional status of 232 nursing home patients (mean age 72.9 ± 12 years) was determined using biochemical and anthropometric measurements. Overall, the incidence of some degree of malnutrition was 59%. Seventeen of the patients were found to have pressure sores and were all malnourished. When classified as mild, moderate, or severe malnutrition, the patients with pressure sores were in the severe group. There was a significant difference (P<.002) between the nutritional status of pressure sore patients and the malnourished patients. It appears that the development of pressure sores correlates with nutritional deficiencies. The authors' findings suggest a need for more aggressive nutritional support in the elderly, especially those with pressure sores. J Am Geriatr Soc 34:435–440, 1

This chart review study describes 40 geriatric hip fracture patients focusing on the in‐home functional and environmental assessment done before discharge from a rehabilitation hospital. Patients were aged 65 to 96 years. Before hospitalization all ambulated independently and three‐quarters required limited or no social support. On discharge, none ambulated independently and all required support. Discharge plans for three patients were changed by the home visit, resulting in two nursing home placements. The role and structure of the rehabilitation home visit are outlined together with the common safety recommendations and equipment needs. The home visit assessment form is presented. The rehabilitation home visit is a practical tool to help achieve successful home discharge for elderly hip fracture patients. It is particularly useful in discharge planning for elderly patients with hip fracture, amputation, or stroke. J Am Geriatr Soc 34:441–447

The aging process is characterized by a decline in the function of many organ systems. Changes that occur in cardiovascular, endocrine, and immune function with aging have been described extensively. The bone marrow clearly plays an important role in normal physiology, being responsible for oxygen delivery to tissues as well as participating in normal hemostasis and host defense. There is evidence that anemia is common in older persons and that the increased morbidity and susceptibility to infection that occurs in the elderly is in part related to altered granulocyte function. This review will discuss normal bone marrow function and describe the most recent evidence implicating alterations in hematopoiesis with age.
