ABSTRACT:Postmenopausal women lose 75–85 percent of estrogen and progesterone production compared with that in their reproductive years. The marked decrease in estrogen results in changes in the uterus, cervix, vaginal lining, glandular breast tissue, and the rate at which bone loses thickness. Some of these changes result in symptoms which range from unpleasant to disabling. Many of the symptoms can be alleviated by treatment with estrogens. However, recent reports have been linking estrogen usage with carcinoma of the uterus. ABSTRACT:To estimate available resources for teaching geriatrics to medical students, a survey of the members of a university Department of Medicine was undertaken. Information was elicited in six areas: 1) personal data, 2) characteristics of each physician's clinical practice, 3) potential interest in a Division of Geriatrics, 4) the comparative levels of aggressiveness indicated in the management of an old patient versus a young one, 5) the attitude of physicians toward the elderly, and 6) a rough measure of knowledge of the demographic characteristics of the elderly. The results suggest that a significant number of faculty members of a university medical school would indeed be interested in teaching and research in the field of geriatrics.

ABSTRACT:This report presents the views of over 1000 administrators, nurses and physicians with respect to the responsibilities, authority and impact of the medical director in a long‐term care facility (LTCF), as well as the nature and future of long‐term care and geriatric medicine in the USA. Nurses, administrators and physicians were more able to agree on the bureaucratic functions of the medical director than on the functions which might be considered more clinical or activist in terms of the total needs of the LTCF. There was no consensus about the external factors which might determine the future of LTC, i.e., alternatives to institutionalization, public attitudes or community hospital relations. Instead, concern seemed to focus upon the “low level” of funding for LTC. With respect to the nature of LTC itself, most respondents felt that the area was clinically challenging. However, only 40 percent of the physicians believed that clinical skills were more important than a sympathetic regard for patients. ABSTRACT:The Department of Family Practice and Community Health of the University of Minnesota Medical School is composed of six affiliated units. Each unit is allied with a hospital complex and operates an independent family practice clinic. Since each unit has unique demographic characteristics, general objectives for training residents in geriatric medicine have been developed. Cognitive, attitudinal and skill objectives also have been identified. The unit director implements the general objectives by using the special strengths of that particular unit.

ABSTRACT:In order to pursue the question of excess mortality due to chronic brain syndrome (CBS), 330 residents of a home for the aged were independently evaluated prior to admission by psychiatrists and by general physicians. Five years later, their medical records were searched to determine: 1) age on admission, 2) diagnosis of CBS on admission, 3) physical status on admission, and 4) if death had occurred, the age at death and the cause of death. Women outnumbered men by 3:1. Subjects with CBS outnumbered those without CBS by 2:1. A previous mortality study on 145 aged subjects at the same institution provided an excellent frame of reference for this investigation. Among the men there was no difference between the mortality rates for those with CBS and those without. Among the women a statistically significant difference in mortality was found between those with CBS and those without. Women with CBS and a “poor” physical status had the highest mortality rate. Those who initially had CBS died significantly sooner even if their admission physical status had been “good.” Their mortality rate exceeded that for women with a “poor” physical status who did not have CBS. Bronchopneumonia caused death twice as often in subjects with CBS as in those without CBS, both among men and women. It is concluded that CBS per se is a significant factor in increasing mortality

ABSTRACT:Modern geriatrics is the product of notable clinicians, past and present. However, recognition has been unequal. Pioneers whose publications were pre‐eminent in their time have not received adequate identification. In consequence, the field is poorer for such neglect. Albert Mueller‐Deham (1881–1971) of Vienna did excellent work in geriatrics. His clinical abilities were based on long staff attendance at a major hospital and by his insistence on attendance, with his staff, at autopsies. His key textbook was published in 1937 in German. His career in Austria was disrupted by political forces but, with energy and ability, he continued it with distinction in the United States. After retirement at age 70, he turned to social writings. A bio‐bibliography of this noteworthy man is based on several letters, a personal narrative that he wrote in response to questions about his years in geriatrics, notes from one of his staff, and recollections by his co‐author in 1942 of the revised American edition of the book. ABSTRACT:The author's concept of loneliness is described, especially as it concerns the elderly. An attempt is made to develop an understanding of the word, with its implications for guidance in the nursing profession. The nurse's defenses against her own feelings of loneliness and depression can inhibit therapeutic interactions with lonely or depressed patients. Appropriate nursing action depends upon the knowledge, correlation and use of many other concepts such as those related to dynamics of behavior, personality structure and development, or problem solving. Practical recommendations for nursing intervention are outlined. Emphasized is the necessity for the nurse to understand the concept of loneliness so that she may render effective service.
