ABSTRACT:This period of rapid change in our society engenders much psychological stress for the elderly. Ways for maintaining mental health in old age are discussed. A fundamental factor in modern management is the recognition that the emotional problems of the aged are the same as those in younger persons; there is no specific pattern after age 65. The preventive aspects of psychiatry should be increasingly stressed.

ABSTRACT:The best method for cardiac pacing is transvenous. The percutaneous transfemoral approach has gained increasing popularity, especially when the objective is to obtain a His‐bundle recording. Although most of the clinical experience on cardiac pacing in the past decade has been with ventricular pacemakers, atrial pacing is becoming an increasingly important clinical tool. Among its many diagnostic, prognostic and therapeutic applications are: stress test for angina, diagnosis of the Wolff‐Parkinson‐White syndrome, recognition of bilateral bundle‐branch block, differentiation between mitral valvular obstruction and left ventricular dysfunction in rheumatic heart disease, selection of the pacing site in the sick sinus syndrome, and safe termination of supraventricular tachycardias in overdigitalized patients. The extensive use of cardiac pacemakers has also resulted in a new spectrum of auscultatory phenomena. Their occurrence depends upon the mode and site of pacing, the nature of the supraventricular rhythm, and the production of extra sounds and murmurs by the pacing catheters. ABSTRACT:The authors interviewed 100 psychogeriatric patients whom they knew well, and obtained their responses to 6 questions concerning what the patient wouldliketo be doing in one year, five years, and ten years versus what he wouldexpectto be doing in one year, five years, and ten years. These 100 cases were subdivided into: Group I, 22 patients who had psychiatric functional illnesses (no organic brain syndromes); Group II, 39 patients who had recovered from their functional illness; and Group III, 39 patients who responded both when ill and when recovered. As a control, 100 geriatric persons who had never been mentally ill were similarly interviewed (Group IV). Except for a negligible number of optimistic replies, chiefly in Group IV, the responses were consistently pessimistic. Apparently the elderly person can develop a “no expectations” mental attitude that protects against frustration and disappointment. This amounts to a premium‐free insurance policy for mental health and may be a reason why the incidence or prevalence of functional mental illness is no greater in the elderly than in the

ABSTRACT:In 79 of 89 patients operated on for severely ischemic lower extremities, the procedure used was femorotibial or femoroperoneal bypass. Two thirds of the patients were over 60 years of age. In some cases the follow‐up period was as long as eight and a half years. A functional bypass was achieved as often in the older groups (67–77 per cent) as in the younger group (74 per cent). The rate of limb salvage was lower in the older groups, but it was still a significant 57–65 per cent versus 74 per cent in patients younger than 60. Femorotibial bypass in lieu of primary amputation is recommended for elderly patients with such absolute indications for surgery as gangrene, gangrenous ulceration, or rest

ABSTRACT:The experience of a centrally administered, neighborhood coordinated, home‐care program developed in Israel as an extension of a Health and Welfare Center for the Aged, is described. During the first two years 85 aged, most of them bedfast and home‐bound, were provided with a variety of services by the home‐care team, either directly or by drawing on the different community resources.Coordinated home‐care programs incorporating an interdisciplinary approach are a valuable and effective component in the community services required for the care of the chronically ill. Such services can help alleviate many of the hardships experienced by these patients and their families. Fewer of these patients need placement in long‐term care facilities, or else their placement can be postponed. The provision of continuity of care helps to reduce re‐admissions to general hospitals, and those admitted have shorter stays, thus permitting a higher patient turnover and better utilization of beds for the care of acut

ABSTRACT:A comprehensive program for geriatric health care is outlined. The complexity of the clinical picture should be recognized as a basis for providing proper management. Special attention should be given to the clinical history and the physical and laboratory examinations in geriatric patients. The results of holistic treatment often are highly rewarding and can do much to modify the persistent false image of old age.

ABSTRACT:An unusual case is presented of carcinoma of the prostate metastasizing to both breasts (verified by biopsy). This is the sixth such case to be reported. The relationship of breast tumors to estrogen therapy is discussed. The estrogen therapy given for treatment of prostatic carcinoma apparently does not influence the occurrence of metastases to the breast.
