AbstractThe author outlines the history of the isolation of therapeutic gases from the ambient air, and also the industrial introduction of gases that pollute the air and menace the health of man. Both geriatric and middle‐aged patients with respiratory and cardiac disease have experienced exacerbation of symptoms and even death as a result of high concentrations of air pollutants.Methods have been developed (e.g., alkaline sprays and air‐filtering units) for combatting emergency periods of air pollution in the home and hospital. AbstractA review is presented of the data on aged patients with thermal burns involving more than 10 per cent of the body surface, treated at the University of Iowa Hospitals during the five‐year period 1965–1969. AbstractClavicular cortical thickness measured in postero‐anterior chest roentgenograms was decreased in 53 patients after gastrectomy as compared with 447 males and 277 females in apparent health and 122 males and 109 females with peptic ulcer not surgically treated. AbstractIn a triple‐blind, crossover, fixed‐dosage study, sodium butabarbital (Butisol Sodium) was tested as an aid to sleep for 53 hospitalized aged psychiatric patients with sleep disorders. After one week of placebo therapy to detect “placebo reactors,” the additional three‐week schedule (oral administration) consisted of the following sequences: (a) placebo, Butisol 50 mg, Butisol 100 mg; (b) Butisol 50 mg, Butisol 100 mg, placebo; (c) Butisol 100 mg, placebo, Butisol 50 mg. Observations were made at the first two half hours after administration of medication at 8 p.m. and hourly thereafter until 6 a.m., for onset of sleep, duration of sleep, number of times awake, duration of waking period, quality of sleep, and mental state on awakening. Three tests were used for evaluating the patients: CGI, NOSIE and TES.Sodium butabarbital (Butisol Sodium) in moderate dosage (50 mg) was an effective hypnotic agent for the majority of the patients and also appeared to be conducive to behavioral improvement in the daytime as reported by both nurses and psychiatrists. There was no advantage to using the 100‐mg dosage as an hypnotic, and some disadvantage with respect to daytime behavior. Patients slept better and longer during the drug periods than during the placebo periods. There were no more side effects during the drug periods than during the placebo periods, nor were more detrimental effects evident upon awakening. AbstractA study was made of the foot problems of 426 elderly patients in the Foot Clinic of Baltimore City Hospitals. The disorders causing foot pain and disability were: circulatory (31 per cent), structural (21 per cent), nail and skin problems (36 per cent), neurogenic (8 per cent), and miscellaneous (4 per cent). Illustrative cases are discussed.The health of the feet is an important factor in the maintenance of the elderly patient's mobility and independence. Early recognition of foot lesions is essential in achieving this goal. In the early stages, simple therapeutic measures (foot hygiene and wearing proper shoes) often provide great relief and prevent further deterioration.
