ABSTRACT:The Inter‐Society Commission for Heart Disease Resources (ICHD) is an organization composed of 150 distinguished practitioners of cardiovascular medicine. In 30 reports it has identified the essential resources required for optimal cardiovascular care. This includes detailed specifications for cardiac surgical problems including catheterization‐angiocardiographic laboratories, coronary care units, pulmonary care units, instrumentation safety programs for hospitals, and many other diagnostic and treatment services. In addition, the Commission suggests ways in which a community's hospitals can be interrelated into a stratified system so that taken together the combination of resources provides the best possible cardiovascular care available under existing conditions. ABSTRACT:Hemodialysis, renal homotransplantation, or both were employed at the Peter Bent Brigham Hospital as treatment modalities for renal failure in 100 patients of the 50–80 age group. Complicating nonrenal disorders were common. Despite the infirmities of advanced age, rehabilitation was achieved in 68 per cent, and the cumulative mortality was only 31 per cent. ABSTRACT:At autopsy, extensive knee arthrotomies were performed on 11 Scottish males (average age 77) and 11 females (average age 75) who had died from various medical disorders and had no history of complaints referable to the knees. The aim was to study the incidence of horizontal (cleavage) tears of the menisci in the general population. In the males, there was a high incidence of cleavage tears of the medial meniscus (in 14 of 22 knees) whereas the lateral menisci were usually normal (in 21 of 22 knees). In the females, there was a low incidence of both medial (in 6 of 22 knees) and lateral meniscal tears (in 2 of 22 knees). Cleavage tears of the medial meniscus in geriatric males may be part of the aging process, and this should be kept in mind if surgery is contemplated for reasons other than severe symptoms.

ABSTRACT:A system of coordinated care for certain disorders by The Jewish Home and Hospital for Aged and adjacent general hospitals is described. Patients requiring surgery, surgical diagnostic procedures, radiotherapy, radioisotopic diagnostic and therapeutic procedures and the insertion and follow‐up of electronic cardiac pacemakers are given inpatient and outpatient care by the general hospital. Coordinated care is insured by full exchange of medical and social information, use of a private ambulance service, free availability of beds and early return to the Home for rehabilitation and after‐care. In most cases, medical care at the general hospital is supervised by specialists with independent appointments on the staff of the Home. In 1970, 199 patients from the Home were given inpatient care in general hospitals; the average length of stay was 11.3 days. Thirty‐nine patients were given outpatient care, with a total of 160 v

ABSTRACT:Two hundred and thirty‐seven chronically sick aged patients were skin tested with Tuberculin PPD, Intermediate Test Strength. Healthy younger staff members constituted a control group. The percentage of negative reactors among the patients appeared to be directly related to the severity of their chronic illness, as represented by the various ward categories. The percentage of negative reactors among the patients who subsequently showed a positive reaction to Tuberculin PPD, Second Test Strength, was directly related to the state of health of the patients. In sick old persons, a negative reaction to Tuberculin PPD, Intermediate Test Strength, should be followed by testing with Second Test Strength before it can be confirmed that the person is a negative reactor. The significance of a negative reaction to even very large doses of Tuberculin PPD should be viewed in the context of the overall clinical picture when dealing with a chronically sick aged patien

ABSTRACT:A 91‐year‐old woman was admitted to a mental hospital, with a primary psychiatric diagnosis. For two years she had had various abdominal complaints and anorexia, and finally had become “grossly disturbed.” However, the physical, radiological and laboratory findings were not conclusive, although a diagnosis of peptic ulcer was entertained at one time. She became almost completely withdrawn. There was no jaundice until shortly before death, which occurred after twelve weeks in the hospital. Autopsy revealed carcinoma of the head of the pancreas with extensive metastases.
