AbstractThe medical profession has participated in many scientific breakthroughs which have prolonged the life‐span of millions of people. The physician, in the daily practice of his art, continues to prolong individual lives. Whether he will or no, the modern doctor therefore finds that he has a continuing responsibility to deal with the social consequences of those historic and current acts. When his patient is elderly and impaired, and when institutional placement must be considered, the doctor has a view from the bridge. His professional and human judgments indicate theoptimumsolution. The current realities dictate the options. He can look towards the general community and press for the development of services to make possible his prescription of continued community living for the patient and he can look towards the institutional community for what is necessary in care and programs to make the lives he has prolonged worth living. AbstractA retrospective study was made of the 243 geriatric patients with herpes zoster and post‐zoster complications referred to Duke University Medical Center during the years 1939 through 1968. The factors of age, race, sex, distribution, laterality, duration, severity, complications, and associated disorders were evaluated. There was an absolute increase in the frequency of trigeminal nerve involvement among geriatric zoster patients compared with zoster patients of all ages. The commonly‐held belief that the duration is longer and the disease more severe in geriatric patients was confirmed. The incidence of complications also was significantly increased in the 243 geriatric zoster patients.Suggestions concerning the management of both uncomplicated and complicated cases of herpes zoster are made. AbstractThe overall mortality in a series of 132 hospitalized patients (43 men and 89 women) with acute pancreatitis was 28 per cent; for patients younger than 60 it was 9.1 per cent, and for those older than 60 it was 41.6 per cent. The typical case with a fatal ending was represented by the aged patient with several diseases, especially heart disease, for whom pancreatitis was a catastrophic stress. Fifty‐one per cent of the deaths occurred during the first twenty‐four hours after the hospital admission. In one‐third of the cases, the pain differed from the usual pain of pancreatic disease; in 18 per cent it simulated pain from cardiac infarction; and in 9 per cent there was no pain. The gall bladder was definitely pathological in 48 per cent of the patients; 18 per cent had undergone subtotal gastrectomy for gastric ulcers; 4.5 per cent had active ulcers; and 9 per cent had diabetes. Palpation findings were negative in 13 per cent. Half of the serum transaminase (SGOT) values and nearly half of the alkaline phosphatase values were elevated. In 38 per cent of the cases, icterus was present in some phase of the disease. Nasogastric intubation was used for treatment in 13.6 per cent of the cases, and enzyme inhibition by trypsinkallikrein inactivator was attempted in about 30 per cent. AbstractThe data on 539 patients (358 men and 181 women) with proved myocardial infarction, admitted to the hospital during a four‐year period, were reviewed and tabulated according to yearly and monthly intervals for both sexes. The admission rates were studied in relation to hypertension, hypercholesterolemia, diabetes, and mortality, and also in relation to the seasonal changes in temperature. The analysis showed that, for men, the curves representing admission and mortality rates were not characteristic. In contrast, for women, there were two distinct peaks of admission, in April and in December; a chi‐square value of 16.10 (df = 11) tended toward a significant difference. A similar tendency toward a significant difference was noted in the increased mortality for women in the spring months of March, April and May; chi‐square value, 4.76 (df = 3).
