ABSTRACT:A comparison was made of the mortality rates for various surgical operations, by age groups and by time periods. The age groups were: under 60, 60–69, 70–79, and 80 or over. The three time periods were: 1951–1955, 1956–1967, and 1967–1977. Only slight improvement was noted in these mortality rates despite the fact that during the 1951–1977 period many improvements were made in anesthesia, surgical techniques, antibiotic therapy, intensive‐care units, and presumably the better understanding of disease. Further improvements will depend almost exclusively upon studying the basic problems of old age, chiefly atherosclerosis with its effects on the heart, blood vessels, brain, respiratory system and urinary tract. Unless this is done, there is no prospect of great improvement in operative results, and no basis for believing that the operative risk in the aged is the same as in the young. ABSTRACT:In order to determine the characteristics of stomach cancer in the elderly, statistical analyses were made of 2,366 cases found in a gastric mass survey (group 1) and 194 cases found among 9.196 persons over age 40 observed during 13 years (group 2). For group 1, the average annual incidence of stomach cancer per 100,000 persons by age, and the death rate by age, increased as age advanced. The average annual incidence was 0.19 percent over 18 years and there were almost no changes in the detection rate. For group 2, the death rate per 10,000 persons in the examined group was significantly lower than in the non‐examined group, especially for persons in their 50's or 60's. However, there were no differences in the death rate for persons in their 40's, 70's, and beyond. The rate for surgical resection of cancer and the incidence of early stomach cancer were lower in the elderly; the 5‐year relative survival rate was poor. Thus, the clinical characteristics of stomach cancer in the elderly include the following aspects: 1) in mass surveys, the incidence of stomach cancer is highest in the elderly, especially for men; 2) many cases of stomach cancer are advanced or inoperable; and 3) most of these patients do not undergo examination until they become aware of symptoms related to can

ABSTRACT:The vitamin profile of 473 elderly persons was compared with that of 204 healthy volunteers (controls). Of the 473 elderly, 327 (mean age 87) lived in nursing homes, and 146 (mean age 77) lived at home. The 204 controls were in the 20–50 year age group. The circulating levels of biotin, pantothenate, riboflavin, vitamins A, B6, B12, C, E, folate, thiamine, nicotinate, and carotenes were determined in all groups. Hypovitaminemia was not obvious for biotin, pantothenate, riboflavin, vitamins A, E, and carotenes in either the institutional or non‐institutional elderly. Thiamine, vitamin C and vitamin B12levels were strikingly depressed in the non‐institutional elderly as compared with the institutional population or the controls. Over 30 percent of the institutional elderly had vitamin B6and nicotinate hypovitaminemia compared to the controls. Both the institutional and the non‐institutional elderly showed depressed levels of folate and vitamin B12. Vitamin supplementation reduced the percentage of folate and B12deficits. The marked vitamin deficits in the institutional population, in descending order of incidence were: vitamin B6, nicotinate, vitamin B12, folate, thiamine, and ascorbate, whereas the pattern for the non‐institutional elderly was: vitamin B12, thiamine, ascorbate, vitamin B6, nicotinate, and folate. In the 473 elderly studied, vitamin B6, nicotinate, and vitamin B12, in that order, were the most common deficits; folate, thiamine and ascorbate deficits comprised a lesser percentage. ABSTRACT:Erythrocyte folate levels were compared among 25 young healthy subjects (mean age, 27 years), 29 healthy subjects aged over 75 living in the community, 62 subjects (mean age, 75) admitted to a geriatric assessment ward (acute illnesses), and 32 subjects (aged over 65) in a ward for long‐term physical or mental illnesses. Overall, the female/male sex ratio varied from 2:1 to 3:1. For the three elderly groups, the incidence of low erythrocyte folate levels (less than 100 mμg/100 ml) were 24 percent, 16 percent, and 18 percent, respectively. ABSTRACT:Fluphenazine enanthate parenterally was compared to an antipsychotic drug (haloperidol) given orally for the “crisis intervention” treatment of elderly outpatients with late paraphrenia. In the fluphenazine group, 11 of 13 patients improved, compared to only 3 of 13 in the oral drug group (p<0.01). ABSTRACT:A 12‐week program for 42 chronically constipated patients in a nursing home involved a graduated dosage regimen of standardized senna concentrate with dioctyl sodium sulfosuccinate (Senokot‐S Tablets), a high‐fiber diet, and an increased fluid intake. The program achieved a satisfactory pattern of bowel evacuation in 36 (86 percent) of these patients. This pattern, as observed during a 4‐week follow‐up period while therapy was continued, was characterized by absence of fecal impactions, regular comfortable bowel movements of appropriate consistency, minimal or no straining at stool, and minimal or no resort to enemas. In the initial 2‐week control period, none of the 42 patients had responded adequately to only dietary modification and increased hydration. Six additional patients whose response to the control regimen was equivocal, also appeared to benefit from the medication program. Two of these were able to discontinue the laxative tablets completely by the end of the 12‐week test period. ABSTRACT:A 40‐item quiz on aging was given to 46 health care professionals, before they participated in a workshop dealing with the care of the elderly patient. In the quiz, they were asked to identify physiologic and functional conditions either as normal aging processes or disease‐related processes. In general the results indicated that disease processes were the easier entities to identify, and that the workshop experience enhanced this effect. ABSTRACT:CNS stimulants such as methylphenidate and pentylenetetrazol are currently prescribed for the treatment of aged persons with symptoms of senility. A review of clinical studies conducted with these compounds indicates that firm evidence of efficacy is lacking. Because of the risks associated with high doses of such stimulants in the aged, it would seem that these compounds are of limited utility in geriatric psychiatry.
