The association of atherosclerosis with brain atrophy was studied in 101 subjects (75 men and 26 women) using computed tomography (CT). A calcification index (CI***) was calculated as an indicator of atherosclerosis in the abdominal aorta: the calcified portion along the entire circumference of the abdominal aorta x*** 80. Both the cerebrospinal fluid (CSF) space volume and the volume percentage of the CSF space to the cranial cavity [brain atrophy index (BAI)] were calculated as indicators of brain atrophy. The CSF space volume increased with increasing age after the 70s. The BAI increased with increasing age after the 60s. Both the CSF space volume and the BAI in the 70s were 1.8 times those in the 40s. The CI increased with increasing age after the 50s very rapidly, and the CI in the 70s was 54 times that in the 40s. Both the CSF space volume and the BAI were greater in atherosclerotic (CI greater than or equal to 3.0) than in nonatherosclerotic (CI less than 3.0) subjects in the 60s and the 70s.

The symptoms associated with acute myocardial infarction in a series of 777 elderly hospitalized patients are reviewed. Their ages ranged from 65 to 100, with a mean of 76.0 years. The spectrum of presentation changed significantly with increasing age. Chest pain or discomfort were less frequently reported, although present in the majority of patients up to 85 years. Syncope, stroke, and acute confusion became more common and were often the sole presenting symptom. Shortness of breath, although the most frequently reported symptom in the absence of chest pain, was equally common at all ages. Thus, in patients aged 85 years or over, “atypical” presentation of myocardial infarction became the rule, and in the very old the clinician must be prepared to screen for the diagnosis in most acutely ill patie

Previous studies have identified postprandial systolic blood pressure reductions in old, frail institutionalized subjects, which do not occur in healthy, young subjects, after a morning meal. To evaluate the relative contributions of state of health and time of day to this potentially dangerous abnormality in cardiovascular homeostasis, we measured sitting systolic blood pressure and heart rate before and at intervals after a noon meal, and in identical fashion without a meal, in 21 healthy, community‐dwelling elderly subjects (73 ± 6 years of age) attending a nutrition program. Systolic blood pressure changed a maximum of −11 ± 9 (SD) mmHg CP = .006, analysis of variance) by 60 minutes after the meal, in contrast to 1 ± 7 mmHg (NS) by 60 minutes, when no meal was given (P<.0001, meal versus control studies). There was a highly significant inverse correlation between postprandial and basal sitting systolic blood pressure changes (R = −0.60, P = .004). Healthy community‐dwelling elderly demonstrate postprandial reductions in systolic blood pressure which correlate with basal sitting systolic blood pressure. The effect of age on glucose tolerance, as differentiated from the effects of obesity, work and leisure physical activity, family history of diabetes, and the use of drugs known to adversely affect glucose tolerance and/or insulin secretion, has been analyzed in 732 factory workers aged 22 to 73 years. Glucose tolerance, as evaluated by the plasma glucose response to 75 g of oral glucose deteriorated with age, associated with an increase in plasma insulin levels. However, the age‐related decrease in glucose tolerance also correlated significantly with degree of obesity, leisure‐time physical activity, and the use of potential diabetogenic drugs. Partial correlation coefficients were calculated to define the effect of age per se on glucose tolerance, controlling for the presence of these other age‐related variables. When this was done, the degree of correlation between age and glucose tolerance was reduced, particularly in women, to where it became of marginal statistical significance. The effect of age on insulin response was affected to a greater degree by age‐related variables, and was no longer statistically significant when these other factors were taken into consideration. The percentage of elderly persons retaining natural teeth is increasing. Although many of these individuals cannot be convinced to see a dentist regularly, they are generally seen by a physician or nurse relatively frequently. The physician or nurse can provide a vital service for these patients by performing oral screening examinations. A technique is described for accomplishing this.

The syndrome of granulomatous angiitis related to varicella zoster virus infection often manifests as herpes zoster ophthalmicus followed by contralateral hemiplegia. Forty‐five cases have been reported to date, and the authors' experience with two additional cases seen in a one‐year period is described. Given the frequency of both stroke and herpes zoster ophthalmicus in an aging population, the authors postulate that granulomatous angiitis is underdiagnosed. A case is presented and acquired factor VIII deficiency is discussed. The studied patient was elderly and had cancer of left breast, cancer of the head of the pancreas, history of blood transfusions, and was on tarnoxifencitrate therapy for breast cancer. Any one of the above factors could have been responsible for production of factor VIII deficiency.
