AbstractTwelve patients with classical idiopathic Parkinson's disease were treated with L‐dopa. All 12 experienced improvement in motor status. In 6, however, marked depressive symptoms developed after several months of treatment. Although the study involved only 12 patients, it reveals a disturbing aspect of the treatment of parkinsonism withl‐dopa. The occurrence of mental depression deprives the patient of the happier existence that increased mobility should offer. There is also the increased danger of suicide; the 2 patients who attempted it were the only ones with a previous history of depression. More studies must be made to determine to what degree mental depression may limit the use of L

AbstractA study was made of the serum insulin level after the intravenous administration of glucose (0.33 gm/ kg) in two groups of normal subjects—18 in the 60–85 age bracket, and 16 in the 23–45 age bracket. All had a normal coefficient of glucose utilization and a normal blood glucose curve after intravenous administration of glucose. Serum insulin was assayed by the radio‐immunological method, with use of an ion exchange resin. Blood samples were drawn at 2, 5, 10, 30, 45 and 60 minutes after the end of the glucose infusion.In the elderly subjects the maximal value was reached only after 5 minutes and the mean value was 39± 9 µU/ml (range, 27–50), whereas in the younger subjects the serum insulin level rose immediately, reaching a maximal value 2 minutes after glucose infusion, the mean value being 54 ± 10 µU/ml (range, 42–66). Thus the response of the pancreatic beta‐cells to functional stimulation in the elderly subjects was slower and weaker than in the younger ones, although there was no decrease in glucose tolerance. AbstractSulfameter (sulfamethoxydiazine, Sulla) is a safe long‐acting sulfonamide of low toxicity. It can maintain blood and urinary sulfonamide levels, with 5 per cent acetylation in the blood and 20–30 per cent acetylation in the urine. The dosage is 500 mg three times daily for the first day, followed by a maintenance dosage of 500 mg daily for two weeks.Good clinical and bacteriological results have been reported from both Europe and the United States in the treatment of respiratory infections, urinary‐tract infections, ear, nose and throat infections, and skin infections including pustular acne vulgaris. Sulfamethoxydiazine also can be used as a suppressive drug for as long as nine months in the management of chronic urinary‐tract infections, with no adverse side

AbstractThe social learning model in contrast to the medical model deserves more trials in preparing schizophrenic patients for release from mental hospitals. Observations by the three disciplines (physicians, psychologists and social workers) on 465 male schizophrenic patients in a Veterans Administration hospital indicated that release readiness was correlated more closely with the degree of social efficiency than with the degree of psychopathology. Thus, in addition to the various treatments used to reduce symptoms, there is great need for resocialization programs.

AbstractA 71‐year‐old diabetic white male was admitted with weakness, weight loss, fever, gastrointestinal symptoms, and postural hypotension. At first he was treated with digitalis and diuretics for congestive heart failure. However, he became worse and increasing signs of adrenal insufficiency appeared although there was no hyperpigmentation. Urinary steroid values before and after administration of ACTH, and the responses to fluorohydrocortisone and cortisone acetate supported the diagnosis of Addison's disease. After initial improvement, myocardial infarction, vasomotor collapse and pneumonia intervened. Despite increased dosages of steroids and the administration of antibiotics, oxygen and vasopressors, the patient died. At necropsy, the adrenals were found to be completely replaced by reticulum‐cell sarcoma.
