This article describes how the Veterans Health Administration’s Community Care Coordination Service developed a technology algorithm to identify patients’ specific telehealth needs and benchmark best practices. Patient satisfaction was extremely high, patients used technology without much difficulty, and acceptance was greater than expected.

Traditionally, it has been difficult to involve physicians in the home care process. Though historically resistant to technology, physicians have begun to accept the Internet as a viable tool for communication. Agencies can leverage this acceptance to make the physician a part of home care, which creates better care coordination, increased regulatory compliance, and improved physician satisfaction.

摘要:
The Cardiac Connection program at Sentara Home Care Services uses two-way video home telehealth to provide comprehensive home care to patients with congestive heart failure. Program results showed an 82% decrease in hospital admissions and a 77% decrease in emergency room visits. In addition, patients felt less depressed, were eating and sleeping better, and felt like less of a burden to their family than before the telehealth visits began.

There is a crisis in the healthcare workforce. Today there are 35 million Americans ages 65 or older, but in less than 10 years, 76 million baby boomers will begin turning 65. Thus, by 2011, at least one in every five persons in our country will be an “older adult” (U.S. Department of Commerce Census Bureau, 2000).
