AbstractA controlled evaluation of a single‐layer everting technique of suture versus a conventional two‐layer inverting technique has been attempted in clinical large‐bowel surgery. Thirty‐five patients suffering from carcinoma or other diseases of the large intestine were allocated randomly to each technique in the conduct of various forms of colonic resection with anastomosis or closure of a transverse colostomy. There were 3 operative deaths after everting suture and 1 after inverting suture, all attributable mainly to anastomotic failure. A higher proportion of patients developed wound infection or peritonitis after everting than inverting suture—51 as against 35 per cent. Overt faecal fistulation was observed in no less than 43 per cent of the patients having everting suture, but in only 8·6 per cent of those treated by inverting suture. Late stricture formation was noted in 2 cases after everting suture, but in none after inverting suture.These experiences are considered to provide a clear condemnation of the use of an everting technique of suture in the large intestine in clinica

AbstractSerial serum cholesterol levels were measured in 50 patients undergoing biliary‐tract surgery and in a control group of 12 patients. There was a transient early fall in serum cholesterol after cholecystectomy alone. The mean maximal fall was 30 per cent, observed on day 6. When the common bile‐duct was drained the serum cholesterol fell a similar amount (36 per cent) but the fall was sustained for longer, the greatest mean maximal fall being on day 9.Loss of cholesterol in the bile of patients undergoing common bile‐duct drainage was measured at and after operation. If the gall‐bladder was not contracted then the maximum loss of cholesterol was shown to occur at operation owing to the high concentration of cholesterol in the gall‐bladder. AbstractSegments of jejunum and distal ileum of varying lengths and different vascular supply were isolated and interposed iso‐ and antiperistaltically. Pressures proximal to the interposed segment were recorded with a fine, soft, open‐tipped cathether introduced via a Mann‐Bollman fistula.1. The distal ileal segments supplied by a single vessel exhibit an unpredictable high degree of tone in some cases, and when these segments were interposed they caused a high degree of partial obstruction irrespective of whether they were interposed iso‐ or antiperistaltically. Distal ilcal segments supplied two or more vessels interposed antiperistaltically caused a degree of obstruction related to their lengths, and interposed isoperistaltically they caused no obstruction.2. Jejunal single‐vessel segments of both polarities exhibited low obstructive qualities. Antiperistaltic jejunal segments of two or more vessels supply behaved in a similar manner to corresponding ileal segments in that the degree of obstruction was related to the length of the segment. Isoperistaltic segments supplied bt two or more vessels caused no obstruction.3. Possible mechanisms accounting for the differing findings in jejunum and ileum are discussed.4.
