AbstractThe anatomy, as demonstrated at operation, has been studied in a series of 100 selective vagotomies. The existence of a considerable number of variations was confirmed. In 33 per cent there was more than one large anterior trunk, but multiple posterior trunks were noted in only 5 per cent. Additional small nerves on the lower oesophagus were reported in 47 per cent of cases. Other variations are described. A technique for selective vagotomy is presented.

AbstractMaximal acid output and insulin tests were carried out in a series of 56 consecutive patients who had either a truncal or a bilateral selective vagotomy and drainage operation.The results of the insulin tests are discussed in the light of criteria established by several authors. The postoperative reduction in basal and maximal acid output was considered, and also the risk of recurrent ulceration.A comparison of the test results of the two types of vagotomy was made, and no significant difference was found.

AbstractOne hundred and seventy‐five cases of acute breast abscess treated surgically have been described. The supposed rationale for the early use of stilboestrol and antibiotics in this condition has been reviewed. Neither stilboestrol nor antibiotics confers any benefit with regard to rate of healing or recurrent abscess formation after incision. Whilst full evaluation of these drugs in breast abscess awaits a prospective trial, it is suggested that, in the case of stilboestrol, there are sufficient potential dangers to make its routine prescription undesirable, if not indefensible. AbstractFine‐needle aspiration of the breast is illustrated, together with a brief review of the development of cytological diagnosis in this field. AbstractMass reduction of an inguinal hernia is a rare cause of intestinal obstruction, and the diagnosis may be overlooked if there is a temporary cessation of symptoms.Two personal patients with this complication are now recorded, in both of whom a previously undescribed sign is recorded, consisting of retraction of the testicle on the affected side into the upper part of the scrotum, tautness of the spermatic cord, and pain deep in the iliac fossa when attempts were made to draw the testicle down.The diagnostic value of this sign is stressed and also its significance in the current controversy as to the exact mechanism of production of this complication.

AbstractThe usual clinical and operative indications for exploration of the common bile‐duct in cholecyst‐ectomies or the selective use of operative cholangio‐graphy in such cases are discussed and compared with 150 unselected cases of cholecystectomy and routine operative cholangiography. As far as calculi in the common duct are concerned, it seems fair to say that routine operative cholangiography demonstrates stones that would otherwise be left in situ and reduces the cases in which exploration of the common duct is needed. In this series, stones were present in the common bile‐duct in 16 per cent; in about one‐fifth of these their presence was unsuspected clinically and at laparotomy. Routine use of the technique is found to reduce the number of false positives.An important secondary benefit of cholangiography is its demonstration of duct anatomy.Various points of technique are discussed, in particular the use of the cystic duct catheter for post‐exploration radiography, drainage, and postoperative radiography.Routine operative cholangiography appears to be a useful adjunct to chol

AbstractOf 9 patients who survived the immediate postoperative period after hepatic transplantation, 6 later died from infection. Pulmonary infection was the major problem. Biliary infection related to bile fistulas may be less in the future with the improved technique of a cholecystdochostomy which was carried out in the last 2 patients. Frequent bacteriaemias were noted and the organisms were often the same as those found in the bile. Most infections were thought to be autogenous. Cytomegalovirus infection occurred in 6 patients, but in none was there evidence that the virus did harm. Broad‐spectrum antibiotic therapy was freely used in the earlier patients but was often followed by superinfection with resistant organisms and fungi.
