AbstractThis study examines the diagnostic accuracy of ultrasonic scanning in the recognition of multinodular goitre in 87 patients referred for examination who were thought by the referring clinician to have a single nodule in the thyroid gland. Twenty‐five were found to have multinodular glands, 23 of which were correctly identified by ultrasound. AbstractWe have measured the rate of gastric emptying of solid and liquid meals by an external scanning technique and have studied pyloric reflux by a radiological technique. Investigations were performed in patients with uncomplicated hiatal hernia, patients with uncomplicated duodenal ulcer and normal controls. There was a significant delay in the emptying of both solid and liquid meals in the patients with hiatal hernia compared with both the duodenal ulcer patients (solid meals P<0·01, liquid meals P<0·025) and with normal controls (solid meals P<0·05, liquid meals P<0·001). AbstractSeventeen patients have completed a double‐blind crossover study of hydrotalcite against placebo in the treatment of bile vomiting after surgery for peptic ulcer. Overall there was no significant difference between the two treatments, with 9 patients improving on hydrotalcite and 5 on placebo. The original operation had been performed less than 3 years before the study in 9 patients; in this subgroup there was an improvement on hydrotalcite treatment in 8 patients but in only 1 on placebo. These differences are statistically significant (P<0·005). Nausea, vomiting, heartburn and epigastric tenderness were improved although gastritis and endoscopic changes were not affected. AbstractAmmonia absorption was studied from Thiry–Vella colon loops in 6 dogs. Four underwent an end‐to‐side portacaval shunt and it was shown that absorption of ammonia from the colon significantly increased postoperatively. AbstractFifteen mongrel dogs underwent a left lateral non‐anatomical partial hepatectomy using a carbon dioxide laser, diathermy or a surgical scalpel. An original prototype of a compact hand‐held laser was used and its effects were compared with those of the other two instruments. An improved dry cut surface was achieved by placing a rubber snare proximal to the planned incision. While no significant differences in the blood loss or in the operating time were found among the experimental groups, the laser beam sealed vessels of up to about 6 mm in diameter without any complementary suture ligatures, which were needed in the other groups. AbstractThe continuity of the T tube tract on removal of the drain was studied by fluoroscopic examination in 51 patients. Leakage was found in 25 cases (abdominal cavity 7, wound drain 6, local cavity 11, combined abdominal and wound drain leakage 1). No correlation was found between the incidence of pyrexia or pain and the presence of leakage. These complications, noted in 10 patients, are probably explained by bacterial contamination of the bile in combination with the trauma caused by extraction of the tube.

AbstractThe results of closure of a loop colostomy presented by various authors are reviewed. It is suggested that postoperative faecal fistula is due most frequently to oedema and constriction at the site of bowel closure. Le Gac's method of closure is described. AbstractOne hundred and two patients with a pilonidal sinus treated by minor excision had a life‐table of 5‐year freedom from recurrence of 89 per cent provided that they attended a dressing clinic until the wound healed. With failure to attend the clinic, the recurrence freedom fell to 57 per cent (P<0·004).
