摘要:
Individuals are able to enter into addictive relationships with many things. Co-occuring with all of the substance use disorders and other harmful compulsions is a chronic love neediness, or lovesickness. Lovesickness is best exemplified by the adolescent in love, although immature love affairs may occur at any age. Successful recovery from substance use disorders may be enhanced throueh the treatment of lovesickness. The treatment of lovesickness requires that the individual develop reliable sources of unconditional love. One source of reliable unconditional-the self-is examined in detail.

Alcoholism and psychiatry have not mixed well. Part of the emerging solution to this problem is a shift from psychoanalytic to existential psychotherapy. This paper endorses the existential approach with alcoholic patients because it focuses on the essential issues of death, freedom, isolation, and meaninglessness. Recovery from alcoholism requires the acceptance of the reality of the fatality of the disease as well as one's own mortality; the ability to make free choices with responsibility, action, and positive and creative will; the capacity to love and be intimate while understanding our basic isolation and separateness; and, the ability to engage life, to make contact with others, and to infuse living with meaning, not absurdity. These more spiritual concerns are the realm of existential psychotherapy.

This paper presents a model of intervention for use with co-dependent women; women with little self-identity and low self-esteem. The model is a structured, goal oriented process which integrates concepts related to co-dependency with techniques of mental imagery. The goal of treatment is twofold. First, treatment should result in improved emotional/psychological/behavioral functioning of the client. Second, it utilizes an intrapersonal model to break the intergenerational transmission of the dysfunctional co-dependent family system, a system in which family members are taught to find personal validation through emotional fusion with others. Phase 1 of treatment focuses on the creation of positive, supportive images and visualizations directed toward the client's self-identity. Phase II expands the client's self-identity and addresses her fears of abandonment. Phase III works toward intrapersonal integration and post treatment personal development. The treatment process is described in detail, and specific examples of images used in each phase are discussed.

A survey of alcoholism counselors from a state and a national sample was conducted as a preliminary study to be used in the planning of a larger survey. The survey attempted to gather qualitative and quantitative data with which to test observations made about the recovering counselor in the alcoholism literature. Recovering counselors were contrasted with non-alcoholic counselors in terms of demographic characteristics and attitudes toward alcoholism counseling as a profession and the issue of relapse in recovering counselors. The results found that despite significant differences in demographic characteristics, recovering and nonalcoholic counselors tended to have very similar attitudes toward issues involving alcoholism treatment and relapse.

Members of a community-based anxiety disorders self-help group were administered the Michigan Alcoholism Screening Test (MAST). Thirty percent of 63 individuals diagnosed as agoraphobic, thirty-three percent of 12 respondents with panic disorder and zero percent of persons suffering from generalized anxiety disorder scored in the alcoholic range of the MAST. The results replicate those obtained from clinical samples of agoraphobics.

Researchers have suggested that alcohol abuse by women may be linked to problems of feminine identity. The subsequent focus of inquiry has been on disturbed feminine identification in female alcoholics and their deviation from sex-role norms. For the most part, the research has narrowly attended to the examination of conflict between levels of sex-role identification, conscious and unconscious, within the woman. The prevailing assumption is that pencil and paper tests can determine the level of masculinity and femininity in our changing social climate and the presence of role conflict. The findings have been conflicting and unclear. This paper critically examines the methodology of five major studies and provides direction for future research.

A sample of 447 patients was selected systematically from 11,323 detoxification admissions in the Massachusetts Alcoholism Management Information system. Client characteristics, readmission frequencies, and post-detoxification treatment were assessed. Most patients (63.1%) had one admission during the seven month study period. The 11.4% with five or more detoxifications, however, required a disproportionate amount of care-44% of the admissions and 40% of the bed days. Individuals with five or more admissions were primarily white males who drank more heavily than other admissions and had more prior alcoholism treatment. Further, they were less likely to enter post-detoxification care following a detoxification. Altogether, 17% of the sample began long-term alcoholism treatment. Clients with two or more detoxifications were most likely to enter a recovery home. Suggestions are made to restructure alcoholism treatment systems and increase the diversity of services available. The proposed changes may reduce the expense of treating chronic readmissions and improve access to long-term treatment.
