The historical relationship between Black alcoholics and the fellowship of Alcoholics Anonymous is reviewed. A detailed analysis of the growth and development of local AA groups in a Black urban community from 1947 to 1984 and the strength and effectiveness of AA as a treatment resource in the Black community. Survey data indicate that the AA program of recovery will work effectively for Black alcoholics regardless of socio-economic status or class. Finally, a hypothesis of cultural affinity is offered as a probable explanation of the cross-cultural success of the AA fellowship with Black alcoholics.

This article addresses the use of spirituality in the assessment and treatment of Black alcoholics and their families who are known to health and human services agencies, including alcoholism-specific treatment centers. Common expressions relating to spiritual beliefs are identified and attention is given to the knowledge needed to formulate an assessment and to make a treatment plan. These include sources of hope and strength, the concept of God and Diety, and the relationship between spiritual beliefs and health. Guidelines for therapists are recommended.

摘要:
In this article the authors present historical evidence that Black patients with organic brain syndromes (OBS) associated with alcoholism are often incorrectly diagnosed as schizophrenic. Case histories of patients with OBS associated with alcoholism who have been diagnosed as schizophrenic are presented. Factors responsible for the misdiagnosis of Blacks with OBS associated with alcoholism are then discussed. Neurochemical evidence is given to support the contention that chronic alcoholism can cause chronic neurochemical brain dysfunction resulting in a psychiatric disorder that imitates schizophrenia. Finally, the treatment implications of misdiagnosis are discussed with emphasis on how misdiagnosis of alcohol-related OBS results in mistreatment and thus a poor outcome for the Black alcoholic patient.

摘要:
Common elements that can be used in an in-patient rehabilitation unit are reviewed. Medical, socio-economic and psychological goals of such treatment are described, with examples of frequently encountered clinical problems and needs. Particular attention is given to problems of Black alcoholics and their unique needs. Emphasis is placed on the need not only for traditional therapy but also a detailed exploration of self-identity and racial or group consciousness as a Black person. African ancestral roots and community connectedness is seen as an important part of the recovery process.

Historical developments and trends of the employee assistance movement are examined and various models of employee assistance programs are reviewed. In addition, the profile of employee assistance clients and personnel are discussed with an emphasis on Black clients, the cultural barriers they bring to employee assistance programs, and the barriers inherent in most employee assistance programs which make them ineffective in reaching and treating Black clients. Clinical and administrative approaches to overcome these barriers are discussed.

A thirteen month retrospective study of alcohol abusing and alcohol dependent patients evaluated by an emergency room (ER) based psychiatric service was completed. The sample of 93 patients included 31 Black patients and 71 White patients. When demographic variables, alcohol history, and final disposition from the ER were studied, the correlation of alcohol history with treatment referral was statistically significant, p<.001. After reviewing the four attitudes toward drinking (Bales, 1946), the history of alcohol use by the Black community was reviewed with implications for treatment. A specific treatment program for Blacks was presented as well as areas for research.

A six-month retrospective review of all patients arriving at a general hospital emergency room (ER) with a complaint involving alcohol use was completed to assess the level of care. Although 28% of all patients evaluated in the ER were Black, 70% of the 870 patients with a chief complaint involving alcohol were Black and Hispanic. Twenty-nine percent (254 patients) of the sample left without being evaluated by a physician. The majority of the sample met the definition of a "skid row" alcoholic. An ER-based alcohol treatment unit was proposed to better coordinate the care of alcohol abusers and alcohol dependent persons. A state statute was identified which facilitates the entry into alcohol treatment of this alcohol-dependent population.

摘要:
This article examines the scope of the multiply addicted involving alcohol and discusses some of the definitional problems of alcoholism. The various patterns of polydrug abuse and its treatment involving alcohol are discussed relative to public health approaches of primary, secondary and tertiary prevention. The need for complete evaluations and comprehensive approaches including social action and community organizations is emphasized. Similarly the need for humanistic and culture sensitive approaches is also emphasized, and the role of the Black church and family are discussed.
