ABSTRACTMalignant neoplasia frequently occurs with Fanconi anemia. Generally this represents hematologic disease; however, solid tumors may also appear. The woman described here developed squamous cell carcinoma of the vulva at age 28, and is the third such case in a patient with Fanconi anemia that has been reported. It is a diagnosis to be considered for the young patient with this type of carcinoma. Increased chromosomal breakage in this syndrome may be important in the pathogenesis of these neoplastic changes.

ABSTRACTThe immunologic reaction to Candida antigens was studied in 188 women aged 18‐61 years. The four groups included 50 women suffering from various vaginal disorders, 50 pregnant women, 38 sterile women and 50 healthy women without candidiasis. The serum antibodies were determined by agglutination, fixation of the complement, counterimmunoelectrophoresis and immunodiffusion; allergy was determined by skin tests. A significantly higher antibody level and a stronger allergic reaction were found in women with candidiasis. The authors discuss determining immunologic reaction for diagnostic purposes and recommend isolating precipitating antibodies and performing skin tests with the mannan‐protein antigens in addition to microscopy and culture.

ABSTRACTCopper‐bearing intrauterine devices are being increasingly used for contraception. A growing number of pregnancies, which may subsequently carry to term, are therefore being exposed to high levels of copper at the critical period of organogenesis, with risk of possible teratogenesis. Two cases of anencephaly occurring in association with these devices are reported.

ABSTRACTThe mortality risk associated with female sterilization in an analysis of 255 812 procedures performed in developing countries from 1973 to 1979 is reviewed. Fifteen sterilization‐related deaths were reported during this period. The overall mortality was 5.86/100 000 procedures. The most common cause of death was the result of anesthetic complications, with infection in second place. The risk of death by procedure was higher with culdoscopy than with minilaparotomy or laparoscopy. The authors believe that low mortality figures are realistic and can be achieved in developing countries when experienced surgical teams are provided with adequate facilities, observe strict medical standards, utilize minilaparotomy and laparoscopy and avoid general and spinal anesthesia.

ABSTRACTFive thousand cases of laparoscopic tubal electrocoagulation were performed for voluntary interval sterilization in outpatient Thai women from January 1974 through June 1978. Immediate complications occurring during surgery resulted from mesosalpingeal hemorrhage, which was successfully managed by omental packing with or without repeated electrocoagulation. The overall failure rate was 0.40%. The authors find this method of tubal electrocoagulation for fertility management to be reliable and safe as an outpatient procedure and to provide rapid convalescence.

ABSTRACTA survey of the use of laparoscopy in rural Thailand was undertaken between June 1974 and December 1977. All the physicians were trained at either Ramathibodi Hospital, Bangkok, or The Johns Hopkins Program for International Education in Gynecology and Obstetrics in the USA; about 30% and 46% of the physicians, respectively, were endoscopically inactive. The complication rate of rural laparoscopy was high and was inversely proportional to the degree of utilization. It is concluded that for a developing country with limited resources, laparoscopic sterilization in rural areas is not cost‐effective; a technique using simpler and hardier equipment should be chosen.

ABSTRACTWhether or not increased prevalence of oral contraceptive use in developing countries is associated with increased morbidity and mortality from circulatory system diseases is a serious concern for health and family planning administrators. An analysis of Taiwan's mortality statistics to determine whether such an association existed was undertaken, taking advantage of the excellent vital statistics and medical care systems existing there. The analysis failed to demonstrate that death rates from circulatory system diseases among Taiwanese women of childbearing age have increased since the promotion of oral contraceptive use began in 1967. In fact, mortality from all forms of circulatory system diseases has generally declined since 1967, and the slopes of declines for females of childbearing age who were exposed to oral contraceptives were somewhat greater than those of males of the corresponding age groups.

ABSTRACTLorazepam, a benzodiazepine derivative, is a hypnotic useful for premedication. Four milligrams given orally, intramuscularly or intravenously, is more effective than many other agents, including diazepam, in attenuating the psychic sequelae of ketamine administration. This study compares the efficacy of intravenous administration of 2 mg of lorazepam with that of 10 mg of diazepam. The results reveal that the lower dose of lorazepam is superior to 10 mg of diazepam in taming ketamine sequelae. Therefore, lorazepam is suggested for use instead of the higher dose of diazepam.

ABSTRACTFifty‐one patients with proven anovulatory infertility were treated for four months. For two months each, a dose regimen of 50 mg of clomiphene citrate daily for five days and one of 150 mg daily for five days was prescribed. During one month of each of these two regimens, each patient also received 5000 IU of human chorionic gonadotropin intramuscularly. Thirty‐three of the 51 patients ovulated at least once during the four months of treatment; the ovulation rate was 64.7%. Sixteen pregnancies occurred during the treatment, for a pregnancy rate of 31.4%. Human chorionic gonadotropin did not appear to increase the ovulation rate.

ABSTRACTA case of a cystic fibrosis patient with two pregnancies that ended in deliveries at 30 and 29 weeks' gestation, respectively, is presented. The author suggests that the increased life expectancy of cystic fibrosis patients because of therapeutic advances will increase the need to examine the association and effects of fertility in individuals with this genetic disease.
