In many cases, traumatic brain injury (TBI) is associated with personal injury litigation requiring rehabilitation professionals to serve as expert witnesses. Increasingly, speech-language pathologists have been called on to play this role. Because of the nature of communication impairment after TBI, the testimony of speech-language pathologists often overlaps with that of neuropsychologists. The purposes of this article are to describe critical aspects of personal injury litigation and expert testimony, describe communication deficits associated with cognitive and psychosocial impairment common after TBI, and promote a collaborative approach to assessment and preparation for legal testimony.

Recognition that traumatic brain injury (TBI) in children has enduring effects on school performance has led to the addition of TBI as an educational disability category. Because many of the communication problems of these children can be attributed to cognitive disruption, speech-language pathologists must broaden the scope of assessment and focus intervention on significant cognitive-communicative and metacognitive needs. Collaboration, including participation of the family and student, is seen as the most sensible and efficient service delivery model for children with TBI. In this article, principles and procedures of collaborative intervention are discussed, and intervention activities that embody these principles are outlined.

摘要:
Communication challenges after TBI are often associated with weak self-regulation of behavior. This article presents an intervention framework within which speech-language pathologists and behavioral psychologists occupy a similar scope of practice in relation to challenging behaviors that are a component of the individual's communication system. We argue for an approach that not only sees challenging behaviors from a communication perspective but addresses these problems in a way that is positive, integrated, functional, collaborative, and environmental. The framework is illustrated by case examples of the authors' collaborative intervention approaches.

This article provides a comprehensive perspective on cross-disciplinary collaboration by the speech-language pathologist and the rehabilitation counselor on removing barriers to employment for survivors of traumatic brain injury (TBI). It emphasizes that for many individuals, the primary obstacles to vocational success are cognitive deficits that affect their capacity for communication. Complementary roles are described for these two rehabilitation professionals in collaboratively addressing these cognitive concerns and others. The role of the speech-language pathologist in job-site intervention is particularly underscored and illustrated through case studies. The success of rehabilitation is based on job-related performance for many individuals with TBI. It should be emphasized that other members of the rehabilitation team, family members/significant others, and coworkers can obviously play significant roles in facilitating a survivor's job-site adaptation, but this article focuses on the rehabilitation counselor's and speech-language pathologist's collaboration, which has not been well described to date.

This article focuses on family involvement as critical to successful rehabilitation of individuals with cognitive-communicative impairments following traumatic brain injury (TBI). A family-centered perspective is embraced within which capacity rather than dysfunction in families is sought. Family members are actively included as collaborators in rehabilitation and reintegration to home, school, work, and community life. This family-centered perspective can help develop capability within the family system throughout its life cycle. Suggestions for understanding family responses to TBI and for involving families more fully are presented.
