摘要:
The purpose of this article is to review descriptive and experimental studies of the conversational turn-management timing behaviors used by children who stutter and their parents. Research findings support the notion that, in general, the temporal aspects of conversations between children who stutter and their parents do not differ significantly from those observed in parent-nonstuttering child conversational dyads; however, for some children who stutter, parental manipulation of factors such as speech rate and response-time latency (RTL) may enhance the child's development of speech fluency. Further, it is apparent that factors such as the age and general speech and language abilities of the child, the severity of the stuttering problem, and the type(s) of disfluencies the child most frequently produces, as well as the difference between the child's and parent's speaking rate, turntaking rate, and so forth, need to be carefully considered when deciding whether to use such therapeutic strategies.

Children engaging in conversations are putting to the test not only what they know about using acceptable conversation rules, but they are also expected to converse fluently. In cases where maintenance of fluency is itself a problem, conversation tasks may prove too difficult to manage fluently. In this article, the connection between conversation demands and the ability of children who stutter to maintain fluency is explored. A model of conversation management useful both in assessment and intervention is introduced. Suggestions for ways studies comparing how children who stutter and children who are normally fluent perform on conversation tasks can be used therapeutically are also presented.

How are language formulation and stuttering related to one another? This article evaluates the extent to which syntactic, lexical, and conversational factors can significantly predict the frequency and loci of stuttered moments in children's speech. A synthesis of the experimental and clinical evidence suggests ways in which the assessment and remediation of fluency disorders in children can be tailored to maximize fluency during the therapeutic process and outside the clinic.

Reports of the prevalence of the co-occurrence of stuttering and disordered articulation/phonology are consistent in the general finding that young children who stutter are more likely to exhibit speech sound errors than children who do not stutter. The extant literature addressing studies of speech sound errors using traditional articulation and phonological process descriptions, which provide information concerning the specific types of errors exhibited by young children who stutter, is reviewed. In addition, some of the approaches that are presently available for treating children who exhibit both stuttering and disordered phonology are discussed.

This article briefly explores current knowledge of how language factors may be related to stuttering in children. Pertinent research and relevant clinical data regarding concomitant speech and language problems are reviewed, along with theoretical models developed to explain this interrelationship. A framework is presented for the differential evaluation of children who stutter that involves four areas of assessment: (1) case history, (2) fluency assessment, (3) language skills assessment, and (4) parent-child interaction, with the greatest emphasis on the third component. Special attention is focused on the identification of language deficits, which may be significant in children who stutter, with consideration for treatment.
