The authors describe how an interdisciplinary team used skills in communication and collaboration to improve patient care on a busy surgical service. A major goal was to establish and maintain continuity of care in the face of decreasing lengths of stay and increasing patient acuity. The authors share their insight about designing and supporting a successful interdisciplinary patient care team and discuss how their experiences relate to concepts such as case management and career development.

Managed care is changing the way in which healthcare delivery is organized and the way in which individuals' basic health needs are met. Although there is great expectation that an increase of primary healthcare providers (physicians, nurse practitioners, physician assistants) will result in better healthcare services, prevailing patterns of health professional distribution raise serious questions about healthcare delivery in rural areas. Nurse executives in rural areas must be alert to emerging needs and organize the nursing delivery system in ways that capitalize on the availability of practicing registered nurses—in the face of shortages of physicians, nurse practitioners, and physician assistants.

Role restructuring can be the key to maximizing efficiency, productivity, and operational effectiveness. The clinical nurse specialist role was restructured from a divisional project focus to a unit-based design to enhance the care of specific patient populations. Should the nurse executive rely on one patient classification model for the allocation of staff and charging for nursing services? In a comparison of predicted patient classification scores with actual scores, patient care needs were underrated by 8 to 33 minutes per patient day, resulting in a deficit of 0.24 to 2.99 full-time equivalent employees. The authors discuss the implications of these findings.

The development of an activity-based management system provides a framework for developing performance measures integral to quality and cost management. Performance measures that cross operational boundaries and embrace core processes provide a mechanism to evaluate operational results related to strategic intention and internal and external customers. The author discusses this measurement process that allows managers to evaluate where they are and where they want to be, and to set a course of action that closes the gap between the two.

摘要:
The staffing of a nursing unit provides a unique challenge when that unit experiences significant seasonal fluctuations in census. Although overtime, per diem, or agency personnel are staffing alternatives to meet seasonal fluctuations in census, they are costly alternatives that are not the most efficient use of human resources. The authors present a seasonal staffing model that reallocates employee work hours in a pattern that closely matches the seasonal work load trends of the department.
