In a study at four teaching hospitals, nurse managers rated themselves most competent in activities related to the management of their unit, and believed these activities were most important. Nurse managers lacked some of the skills and knowledge needed to relate to the larger organization and external environment, activities that they rated as less important. Selection, education, and development efforts should be targeted to newly important roles and skills for which nurse managers have little preparation.

Primary care, with a community-based orientation, is the driving force of healthcare reform. To prepare nurses to provide leadership in this changing healthcare environment, the Manager as Developer Model, useful in the acute care setting, is discussed as a useful model for primary care. Part 1 describes the Manager as Developer Model of Leadership Preparation with its seven major concepts as thje basis for development of professionals. Its application to nurse managers in acute care settings is supported by research studies undertaken in those settings. Also discussed is how the model is used as a process framework in a nursing administration curriculum and the research to date, with its application. In part 2 (October 1996), the authors predict how the model's use can guide the development of leadership and education for the future of community-based practice.

Nurse administrators are seeking ways to create organizational work environments that empower nurses to exercise more control over the content and context of their practice. This study used Rosabeth Kanter's Structural Theory of Organizational Behavior to examine relationships between staff nurses' perceptions of work empowerment, and control over nursing practice, and subsequently, job satisfaction and perceived work effectiveness. Implications for nurse administrators are discussed.

A self-directed work team structure was implemented with a group of staff in a large home healthcare agency in Northern California, with positive outcomes. Managing the troubled or conflicted work group can occupy a great deal of a manager's time and energy. One hundred fifteen nurse managers and administrators responded to a descriptive survey exploring the characteristics of the culture of troubled or conflicted work groups. Those managers and administrators also described numerous successful strategies that they had used in managing these work groups.
