摘要:
The Council on Graduate Education for Administration in Nursing (CGEAN) was established to further develop and improve graduate education for administration in nursing. The Council seeks to identify the nature and direction of education for administration in nursing in various healthcare systems, providing guidelines for programs offering administration. A major goal of CGEAN is facilitating dialogue between nursing service administrators and graduate-level educators who are engaged in teaching and research related to administration in nursing. This column, sponsored by the members of the Council, analyzes and responds to position statements and trends related to the delivery of health services and graduate education for administration in nursing

摘要:
Methods to improve the quality of care are a national issue for home healthcare agencies. In comparison with the traditional process-focused care plans, outcome-focused care plans (OCPs) resulted in significantly belter quality indicator scores for clients cared for by agency staff members. Although OCPs are a valuable tool for enhancing quality, tools are only as good as the individuals who use them. Before deciding to change to an OCP format, administrators must assess all resources needed to effect the change.

摘要:
Codependence in a staff nurse affects both direct patient care and relationships with coworkers, physicians, and supervisors. Codependent behaviors negatively impact the nurse's sphere of influence. The nurse manager's codependency has the same impact, but the sphere of influence is larger and thus, the potential for harm is greater. Codependent behaviors enacted at different levels of an organization can disrupt an entire institution or profession. The author describes how nurses' practices are affected by codependent behaviors and identifies ways in which those behaviors can be modified by good management.

摘要:
Nurse executives have been striving for the means to equalize the power base in healthcare settings. They also have been seeking ways to overcome the perceived powerlessness of their staffs. Empowerment and increased autonomy in the work setting often are described as mechanisms to achieve these goals. This study, based on Ranter's theory of the structural determinants of behavior in organizations, examined formal and informal power as predictors of job-related empowerment and autonomy. Results of the study lend support to the use of the theory in healthcare settings as a way for nurse executives to develop increased access to the sources of job-related empowerment for their staffs.

摘要:
Although the popularity of unlicensed assistive personnel (UAP) models is evident by their implementation across the country, there has been very little scientifically based evaluation of their effectiveness. To facilitate better program evaluation, the authors review the current literature and outline a variety of data elements essential to effective UAP model evaluation. Major concepts that are addressed include program cost-effectiveness, customer satisfaction, patient outcomes, and evaluation of the success of both the training and implementation processes. Research questions that are pertinent to the model and need investigation also are presented. It is only through more thorough program evaluations that nurse executives can make informed decisions regarding the outcomes of care delivery restructuring efforts.

摘要:
Case management plans have been used as a strategy for quality improvement and cost containment of healthcare. The authors discuss the methods used for evaluating the effectiveness of these plans. Variance analysis and measuring expected outcomes of care are the main themes. The authors also present one hospital approach to outcomes management that can be used by healthcare administrators to evaluate patient-care outcomes.
