摘要:
Most managers have only a vague idea of how their employees spend their time. Work sampling is a technique that can identify the types of activities that staff perform and the amount of time that they spend on each activity. With this information, managers can make decisions regarding allocation of staff and activities, thereby increasing efficiency and effectiveness of personnel. This paper describes the use of work sampling as a technique to study employee time allocation and productivity. It offers suggestions for planning, implementation, and utilization of the results of a work sampling study. A work sampling study conducted at the University of Michigan Medical Center's Psychiatric Nursing Department is used as an example.

This study demonstrates the feasibility of determining total nursing costs by DRGs and reinforces the findings of previous studies using the patient classification system methodology. Study implications regarding population trends, inequities of nursing resource use by DRGs, and outliers are discussed also. A final note warns of the potential liability of isolating nursing costs.

摘要:
Eight years of experience with primary nursing in a 750-bed acute care teaching hospital revealed several persistent problems: (1) isolationism among nurses, (2) communication problems between nurses and physicians, (3) inadequate documentation, and (4) insufficient time for patient and staff education, resulting in frustration for the nursing staff. By combining the basic elements of primary and team nursing, a 3-month pilot in one unit resulted in improved job satisfaction for professional staff, improved quality assurance scores, and increased physician-nurse communication.

The results of a survey conducted on the practice of using physician-hired nurses in hospitals are reported. Nursing administrators need information on the nursing services that are being provided in direct association with the medical practice of physicians, particularly when those services influence the quality and cost-effectiveness of patient care. The information can be used to plan more effective approaches to the delivery of nursing health care services.
