摘要:
The ill elderly are more at risk for recurrent hospitalizations than any other segment of the population. What is known about hospital admission/readmission of chronically ill older adults and how can this information be used to develop cost-effective strategies? The authors found variation in percent reimbursement (43% to 93%) of overall/average charges, which indicates that further examination of low reimbursement major diagnostic categories can be useful. Hospital readmission data should be used to improve quality of care while containing hospital costs.

Helping staff adapt to changing organizational and role demands is a key responsibility of nurse leaders. Using Benner's philosophy that identifies the importance of acknowledging one's accomplishments and being acknowledged by others, the authors present the design, implementation, and evaluation of a Professional Nurse Advancement Program (PNAP). The PNAP, offering a single job description that differentiates among three levels of horizontal advancement encompassing four domains of practice, is used in both the promotion and performance appraisal process.

摘要:
Changes in the healthcare environment provide the impetus to look closely at the way in which patient care is delivered. Complex health problems, requiring the expertise of multiple healthcare specialists, coupled with complex hospital systems, result in fragmented care that is confusing to patients and their families. Case management by nurses working closely with physicians in hospitals is one strategy that can reduce fragmentation of care. Case management also facilitates the delivery of cost-effective, patient-focused care. Because of his/her expertise, the clinical nurse specialist (CNS) is an ideal choice for the role of case manager. The authors describe the role transition of nurses from CNS to CNS/case manager over a 2-year period and identify the key system supports required to ensure successful evolution into this expanded role.

The North American Nursing Diagnosis Association classification is approved for nursing diagnoses; the Nursing Intervention Classification system provides a common nursing intervention language but, to date, there is no complete classification for patient outcomes. An outcome classification system can be used for making effective decisions. Recognizing the need for standardized outcomes, clinical nurse specialists developed a classification of patient outcomes. These outcomes, written to fit the Nursing Intervention Classification taxonomy, incorporate Orem's Self-Care Deficit Theory.

The nursing profession is a helping one. However, a desire to “help” by speaking on an issue of concern may result in a lawsuit. In many such cases, the “helper” invokes free speech protection through the Constitution. Nurse administrators have been involved both as plaintiffs and defendants. An understanding of First Amendment free speech protection is vital to nurse administrators in determining whether their own speech is protected and in handling situations in which the nurses they supervise have spoken. The author discusses relevant cases and suggests ways in which nurse administrators can evaluate situations in which they become involved.
