摘要:
This article focuses on the cost impact of patient acuity on DRGs. The traditional method of routine charges for nursing services is compared with the development of a variable charge structure based on patient classification. Examples of each method are given. The authors also analyze a charge structure based upon patient care requirements and apply the budgeting process to the cost determination of nursing services.

摘要:
Unless nurses describe and quantify their contribution to patient care, their financial solvency is seriously threatened under DRG reimbursement. Results of a study that quantified the nursing care of two DRGs using a patient classification system are presented. A description of how one nursing department plans to use this system on a hospital-wide scale is discussed. This information gives nurse administrators an objective means for appropriation of funds, projection of staffing needs, and ultimately, a means of earning revenue.

摘要:
This article is the second in a series of three articles that describe a project set up to define, design, and try out tools for the selection and development of nurse managers. Through the Prospective Managers Pilot Project, Stanford University Hospital nurses have explored an alternative to the traditional approach of ‘working your way up the ladder’ to management competence. Selected candidates, with less than 1 year of clinical experience, strove to learn and demonstrate 25 defined skills of a head nurse. The first article in the series addressed the experiential interview approach to selection of candidates. This article defines the competency expectations of the head nurse, methodology to attain these skills by the prospective manager, and the tools that the project coordinator and mentors used to measure the development of the prospective manager.

With the elderly population increasing, the number of elderly clients served by almost any type of health care organization will continue to increase. What kind of planning does a hospital nursing administrator need to do to assure that her or his nursing service can give better care to a larger numbers of elderly clients? What are the staffing considerations? Services and facilities considerations? Staff education needs? Budget considerations? Quality-of-care issues? This article will discuss these questions and present Eitel Hospital's comprehensive approach to meeting the needs of the hospitalized elderly.
