AbstractDistribution of health problems encountered by nurse practitioners and physicians in general/adult medicine clinics was studied. Proportional samples by clinics were selected from a total of 13,039 patient visits made to four clinics during an 18‐week period in 1978. A Patient Encounter Form was used as the instrument and the ICD‐9‐CM as the coding system. Results showed that (a) 25.1% of the 13,039 patient visits were made to the nurse practitioners and 74.9% to the physicians, (b) 23 frequently reported health problems constituted more than 50% of all health problems seen by both provider groups, and (c) health problems with V codes constituted less than 30% of all health problems seen by both provider groups. Many similarities in health problems seen by nurse practitioners and physicians may indicate that nurse practitioners in the general/adult medicine specialty are used more to substitute for physicians in managing selected health problems than to complement medical pra

AbstractThis study was conducted to obtain information regarding sexual adjustment following ostomy surgery and to explore the influence of surgery on an individual's body image. Home interviews were conducted with 40 subjects, where the mean length of time since surgery averaged 4.6 years. Retrospective perceptions of attractiveness decreased in the first year after surgery, then continued to improve with time. Most comments relative to body image during the first year after surgery had a negative connotation. Persons who saw themselves as most sexually attractive at the time of interview were most often female; however, males had seen themselves as more sexually attractive than females before surgery. Management problems significantly decreased feelings of attractiveness no matter when they occurred. Those with ileostomies who had been ill for over 10 years preoperatively tended to have enhanced feelings of attractiveness at the time of interview compared to those with other types of ostomy surgery and lengths of illness.

AbstractThe relationship between the structure of nursing care and patient satisfaction is examined after equalizing the quantity and quality of nursing staff. An all‐registerednurse model of primary nursing is compared to team nursing. The nursing competency scores of registered nurses (as measured by the Slater scale) on two patient care units were equalized through 6 months of continuing education and staff development programs. A patient checklist was used to determine satisfaction with care and perceived omissions in care. No significant differences in satisfaction with care were found between the two units. The unit with primary nursing was perceived as having significantly higher omissions in care in three categories: dietary needs, reaction to therapy, and contact with nurses. The results suggest that the effect of nursing care structures on patient perception of care may be contingent upon the efficiency of the support systems and the competency of the nursing staf

AbstractCommunity‐based psychiatric treatment and domestic crises have precipitated police officers to intervene with emotionally disturbed people. The investigator hypothesized that an 8‐hour educational seminar would significantly improve knowledge and attitudes of police officers toward people experiencing emotional difficulty. A nonequivalent control group pretest‐posttest design (controln= 33, experimentaln= 27) was used. A knowledge measure specific to seminar content, the Osgood Semantic Differential, and an evaluative questionnaire were the outcome measures. Police officers' knowledge about working with people experiencing emotional difficulty increased, but attitudes were not altered. All officers reported that the seminar was a valuable experience. AbstractCardiovascular responses of nurses were measured during two communication tasks. In the first experiment 40 nurses' blood pressures and heart rates were taken pre, during, and post change‐of‐shift report. In the second experiment, 30 nurses spoke to an individual nurse and to a group of nurses. Blood pressures and heart rates were recorded pre, during, and post speech in both conditions. Analyses of variances showed that speaking levels of all cardiovascular parameters were higher (p<.001) than resting levels in both experiments. AbstractFour studies in which a patient self‐disclosure questionnaire was developed and tested are reported. The instrument was designed to include only content judged by both patients and clinicians as important. Three areas of patient difficulty in disclosing to a health care provider were measured: personal problems and feelings, responses to health care, and life style. Patient difficulty in disclosure was assessed by 21 7‐point rating scales, 7 for each disclosure area; ratings were summed to obtain scores. A correlation matrix of the ratings of 216 patients was subjected to a three‐factor principal components analysis with varimax rotation; the solution accounted for 59% of the total variance. High internal consistency and test re‐test reliability for the total scale and subscales were obtained; Cronbach's alpha .82–.83,n= 214;r= .81–.87,n= 66.
