Nursing in the United Kingdom is currently taking decisions that will enable it to attain mature professional practice – or fail to. It is argued that professions are designed to serve vulnerable people competently and that competent nursing is not reliably achieved by the untrained or those in training. Competent nursing requires qualified nurses who can work in partnership with patients and help them manage their own health and an educational system which facilitates both the initial and continuing mastery of nursing knowledge and skills. Aspects of learning, including the clinical environment, are discussed. The contractual relationship between nurses and doctors is discussed and the need to develop competence in inter‐professional work. Developments in clinical nursing since 1970 are reviewed including beliefs about nursing, models for practice, the organization of nursing, the development of clinical research and the demand for new knowledge and skills from qualified nurses. It is concluded that much of the essential thinking about the nature of professional nursing has been successfully achieved and that nursing in the United Kingdom should now take the decisions and actions to achieve a professional education and professional nursing pract

Nursing is commonly characterized by the process of caring in the context of nurse‐patient interactions in clinical and non‐clinical settings. This interactive context of nursing however should not be seen to only involve encounters of an interpersonal nature but also must be extended to include interactions set in a social‐relational context. (The growing realization that most health problems in our society cannot be fully understood as only an individual phenomena lends support to this notion.) Continuity and quality of time are described as being important aspects of nursing, which make possible a holistic and unified approach to care that is quite unique to nursing. A patient‐centred approach to nursing is contrasted with an illness/disease‐based approach. Growing recognition of the positive outcomes of an approach to nursing which is responsive to patients (rather than primarily focused on illness/disease) suggests that patterns of nursing care should be more closely aligned with this model. The authors believe this can be accomplished through a supportive emotional atmosphere of interpersonal and social‐relational involvement. This paper discusses the goals and strategies used by nurses to achieve professional status. It describes the several interest groups in nursing and identifies the plurality of their goals and the sometimes conflicting strategies used by them. The paper illustrates the constraints experienced by the Royal College of Nursing in policy making and proposes a pluralist structure as a basis for future planning.

摘要:
In the nursing literature trust has been identified as a foundation for a therapeutic relationship between nurse and health care consumer. It is through such a relationship that nurses can help effect needed changes in an individual's patterns of health behaviour. Several authors have described when trust is occurring, but few have attempted to define trust. In this paper trust is analysed, its critical attributes identified, and a theoretical definition given. Model, borderline, related, contrary, and illegitimate cases of trust are constructed. Antecedents, consequences, and empirical referents are also presented. The writer also makes recommendations regarding further study.

A report on recent clinical work in group anxiety management was carried out by nurse therapists whilst undertaking the Joint Board of Clinical Nursing Studies Course 650, Adult Behavioural Psychotherapy (Short Term) in the Plymouth area, England. The group treatments described were based upon a three systems model of anxiety, and aimed to modify cognitive, behavioural and physiological deficits and excesses. The methods used and some collation of measures is shown, followed by discussion of the difficulties encountered. This discussion will relate to: the uses of measures; style of presentation of material, particularly in relation to cognitive change; and adaptability and scope of such groups.

Sickness absence has been a long‐standing concern of the nursing profession. In this paper, sickness is construed as illness or the form a complaint takes. Absence is formulated as a form of coping. A longitudinal study using an experimental and control group design was carried out with student nurses using autogenic regulation training (ART). Comparisons of the relationship between ART and sickness absence records were recorded for: total days off; certificated and uncertificated days off; uncertificated days off; certificated days off; total blocks and total wards; first and second ward allocations; and average time sick. It is argued that ART helped significantly to reduce overall sickness absence in the experimental group when compared with the control group subjects. Reasons for this are discussed and issues for further research are raised. This paper describes the use of the MCR FIRST score. The score has evolved as a method of measuring certain aspects of a mother's caring for her newborn baby. The factors included are feeding, interest, response, speech and touch, and these have been combined into a numerical score. The results for 100 mothers who were assessed on days 1, 3 and 5 postpartum are presented. This paper examines the phenomenon of family ‘interference’ in the health care of a sick member from a new angle. The ‘interfering’ behaviours are located within the context of the family's evolving relationships with health care providers. The authors argue that the behaviours are an understandable product of the family's disillusionment and dissatisfaction with their health care relationships. As such, these interfering behaviours represent the family's most productive means of positively influencing their sick member's experience with illness. In addition, they set the stage for negotiation to take place between the family and health care providers which leads to mutually satisfying care. When interference is appreciated within the context of health care relationships, it becomes obvious that some of our traditional nursing responses are counterproductive. This paper describes the educational activity of a group of 149 nurse teachers and administrators following completion of the nursing education and nursing administration certificate courses of the University of Edinburgh (1958–1975). The data were obtained by a postal questionnaire and three particular types of courses reported by the respondents are discussed. Emphasis is given to the analysis involving degree courses as this was a prominent educational activity, especially among nurse teachers.
