While there has been considerable debate about future roles for public health nurses, there is little research that explores public health nursing from the practitioner's perspective The findings reported in this paper are part of a larger study that explored public health nurses' perspectives and experiences of their practice what they do and how they feel about what they do Qualitative data were gathered through in‐depth individual and focus group interviews with 28 female public health nurses (PHNs) in Alberta, Canada This paper describes how PHNs feel about their work The analysis revealed that public health nurses perceived that their work was valuable and worthwhile, enjoyable, demanding, and not well understood by others These perceptions are discussed in terms of their implications for the future role of public health nursing in a reformed health care system and for the quality of nursing work‐

In an 850‐bed Dutch hospital a study was earned out to evaluate the effects of the implementation of a Dutch form of primary nursing on five job characteristics (viz autonomy, responsibility, feedback/clarity, complexity/difficulty and Job demands) and on the communication and power processes in the organization of the nursing unit The study consisted of three measuring moments one preintervention (t1, March 1992) and two post‐intervention moments (t2, November 1992&t3, May 1993) Primary nursing was introduced in the two experimental units (the experimental group) after t1 No changes were made to the control group After t2, this quasi‐experimental design changed, because primary nursing was also implemented in the three original control units (the control group) after this moment The research variables were measured by means of questionnaires No significant effects were found for the five job characteristics Only the improvement in the communication with the team head and the decrease in the influence of physicians on patient care in the experimental group and the improved communication with physicians in the control group were in line with expectations Changes in the amount of influence of different groups on patient care were inconsistent with what had been expected according to the nurses the influence of the nursing management increased, whereas the influence of the nurses decreased and the influence of the patients did not change In other words, the shift of power was not in the expected dir

This study was designed to investigate differences between cultural groupings in rheumatoid arthritis (RA) The sample was made up of 235 individuals suffering from RA from four groupings Canadian aboriginals, Finnish Canadians, Italian Canadians and other Caucasian Canadians The study was a retrospective chart analysis (non‐randomly selected) designed to compare ethnic groupings on the following dimensions demographic factors, clinical manifestations of RA, laboratory results, and drug therapy response Results of the study support to some extent findings from previous research Specifically, the following was found The study showed a female to male predominance in sufferers of RA in all but one ethnic group Canadian aboriginals tended to contract the disease much earlier in life than other ethnic groups More Canadian aboriginals reported a family history of RA than other groupings Only Canadian aboriginals and Caucasian Canadians showed involvement of the C1‐C2joint with subluxation Italian Canadians tended to have a higher mean haemoglobin than other groupings, while Canadian aboriginals tended to have a higher mean platelet count than other groupings, and an elevated anti‐nuclear antibody count While most of the drugs used to treat RA produced no significantly distinct reactions, the Italian and Canadian Caucasian groups had the highest percentage of subjects with reactive derm

In this study, undergraduate nursing students' self‐efficacy for carrying out health promotion activities with clients in nursing settings was examined Students in the first, second and fourth year of a baccalaureate programme felt moderately efficacious about their knowledge and abilities for health promotion counselling in the three content domains measured (smoking cessation, nutrition and exercise) Senior nursing students had the highest self‐efficacy scores, suggesting a positive impact on health promotion skills over the educational experience In all areas of health promotion, regardless of year in then: educational programme, students reported significantly lower efficacy for their ability to engage clients in an educational programme for behavioural change Educational directions from Bandura's theory are descr

The management of clients who present at the doctor's surgery with leg ulcers follows an accepted but often perilous course It is generally accepted custom and practice that the overall care of this client group is delegated to nurses without the medical practitioner first determining the nature of the underlying pathology The usual arrangement of care for patients with leg ulcers is such that the development of chronic wounds is almost inevitable The author, drawing on the medical and nursing literature and on the findings of her ongoing doctoral research, suggests that nurses in primary care should re‐examine this area of practice and consider alternative approaches that would emphasize the unique contribution professional nursing practice could bring to this patient gro

In order to secure professional standing, nursing has sought to discover and prove the value and uniqueness of the nursing contribution to patient well‐being A prominent feature of this quest has been the assertion by nurses and nurse writers that the nurse has a specific and special function as patient advocate There is a substantial amount of literature to support this view, which is further reinforced by the United Kingdom Central Council (UKCC) for Nursing, Midwifery and Health Visiting which states that advocacy forms an integral part of the nurse's duty to patients This paper provides a critical analysis of some of the features central to advocacy in order to promote clarification of the concept These include the patient's rights and interests in health care, the moral status of patient autonomy, the obligations owed to patients by nurses and the work of independent advocacy schemes It is suggested that the literature tends to confuse advocacy with beneficence which dilutes the significance of advocacy in health care, and that nurses have no special function as patient advocate Furthermore, due to the rigorous demands of the advocate role, the UKCC neither can nor should impose such an obligation on its members, who would be better served by improved support when they endeavour to discharge the full range of beneficent obligations owed to patients within their ca
