As the largest group of professional workers in the health care field, nurses possess the numbers to command a key role among health professionals. It is, however, apparent that nurses must assume a more visible and vital leadership role in the delivery of health care if nursing is to remain a viable profession. Nursing's immediate goal must be to unite forces in an effort to bring about greater involvement of nurses in planning, delivering and assuming accountability for the nursing component of health services at all levels. The ease with which the profession is able to accomplish this goal is dependent upon the ability of nursing's leadership to evoke a sense of harmony and balance within the profession, while creating a dynamic image of nursing among other health professionals and the public.Today, there is a need for strong moral leadership in the nursing profession. In ‘moral leadership’ the power of the leader is used to work with the group as a whole, while guiding individual members towards satisfaction and self‐fulfilment. A critical evaluation of the profession's leadership capabilities should begin with definitions of the role and responsibility of nurses for leadership in the professional association. By its very nature, the professional association stands as the leadership mechanism of the profession and, by working through the association, nursing leadership can unify nursing forces, implement plans aimed at involving more nurses in health planning activities, and facilitate the involvement of individual nurses in determining the conditions of employment under which they pra

This paper describes how the author obtained information in 1972 from a small pilot study on the role and functions of hospital unit nursing officers, in an attempt to challenge his own opposition to the creation of clinical nurse specialist/consultant posts. Data was obtained from an analysis of comments made and questionnaires completed by 40 nursing officers, from the same hospital nursing organization, at a review study day organized by their chief nursing officer. Later in 1973, other information about clinical nurse/specialist/consultant posts, and about unit management schemes, was obtained during a 15‐week study tour in North America. The author illustrates how the information, gleaned from both the pilot study and the North American tour, was used as a basis and rationale for a new nursing service organization, introduced in the Brent Health District following the reorganization of the National Health Service in 197

Patient care can be regarded as a complex activity, which is the outcome of negotiation amongst a large number of different health care professionals who may, at times, hold conflicting views on goals and priorities. If a high standard of care is to be achieved and limited staff resources are to be used efficiently, a strategy of ‘integrated team working’ should be adopted. For the nurse to play her part in the patient care team, in coordinating ward work, carrying out nursing care, monitoring patients' conditions, helping the patient to make sense of what is happening to him, making the most of the ward's opportunities for learning, and so on, she needs to understand the part played in patient care by other health professionals. The study reported here found that amongst the 46 nursing staff of three wards of different specialties, the majority of trained staff did see themselves as having regular working links with other health care professionals‐specifically doctors, physiotherapists and social workers. Only a tiny minority of the 31 student and nursing auxiliaries felt that they had any working links with other health professionals. Most students believed that for the sake of good patient care and effective learning, they should be part of a multidisciplinary team rather than an exclusively nursing team. Whilst the ward manager can do much to foster the development of a multidisciplinary perspective, it is suggested that structuring the workflow along the lines of patient‐centred rather than task‐centred allocation, automatically builds in face‐to‐face interactions between nurses of all levels and other health professionals. This facilitates the development of mutual role knowledge and effective integrated team working. The establishment and development of 3 new university hospital and medical school has obviously affected nursing and nurse training in Nottingham. One result has been that the school of nursing has been able to appoint a community care tutor to specifically teach community aspects within the basic general, psychiatric and paediatric nurse training programmes. This article describes the development of various courses to meet the General Nursing Council for England and Wales (GNC) syllabus objectives. The various community services involved in the course programme are identified and their contributions in time and expertise evaluated. Some discussion is included on the complex relationships between theory and practice and the attempts which are being made to include teaching and learning objectives related to the nursing process within the curriculum. Finally, it is hypothesized that comments from a group of students on their attitude change as a result of the course and, on the value of their new knowledge and skills when they return to hospital, could form the basis of future research.

摘要:
There is considerable evidence to suggest that despite the availability of free medical services under the British National Health Service, there remains a clear social class bias in accessibility to a wide range of health care services. The literature on social class and the use of health services is examined and it is suggested that the greatest inequalities are to be found in the use of preventive services. In the second half of the paper, some tentative proposals, which might erve to reduce existing patterns of inequality, are examined.

This paper describes the establishment and organization of the Joint Board of Clinical Nursing Studies. It outlines the philosophy which has developed since 1970 and which has formed the basis of the Board's work. The belief that ongoing clinical education should contribute to the quality of patient care, and to the personal and professional development of the nurse and midwife, has been fundamental to the formation of the Board's policies. Factual information is given on various aspects of the work of the Board. The difficulty of assessing the value of the work is recognized, but the importance of this activity is stressed. Formative evaluation is required as an integral part of every course. An attempt is made to assess the value of the Board's work to the patients, the nurses taking the courses, and to the nursing service.
